血尿素氮、血肌酐、血清胱抑素C及24h动态脉压对老年高血压患者早期肾损害的临床评价

摘　　要：目的探讨老年高血压患者血尿素氮(BUN)、血肌酐(SCr)、血清胱抑素C(CysC)及24 h动态脉压(24 hAPP)变化特征及其对早期肾损害的评估价值。方法将2015年7月至2016年12月该科收治的老年高血压患者142例按年龄分为老年高血压组(A组,60~<80岁,96例)和高龄老年高血压组(B组,80~92岁,46例)。分析2组患者BUN、SCr、CysC、24 hAPP水平变化。结果2组患者BUN水平变化比较,差异无统计学意义(P>0.05)。B组患者SCr、CysC、24 hAPP水平均明显高于A组,差异均有统计学意义(P<0.05)。结论老年高血压患者SCr、CysC、24 hAPP随增龄而升高。CysC、24 hAPP可作为老年高血压患者早期肾损害的评价指标。Objective To explore the characteristic and clinical signification of the blood urea nitrogen(BUN),serum creatinine(SCr),circulating cystatin C(CysC)and 24 h ambulatory pulse pressre(24 hAPP)in the elderly patients with hypertension of early renal damdge.Methods A total of 142 elderly hypertension patients admitted to the hospital from July 2015 to December 2016 according to their age were divided into elderly hypertension group(group A,60-<80 years old,96 cases)and elderly hypertension group(group B,80-92 years old,46 cases).The changes of levels of BUN,SCr,CysC and 24 hAPPwere analysed in the two groups.Results There was no significant difference between the two groups in the BUN(P>0.05).SCr,CysC and 24 hAPP in group B was higher than those in group A,the differences were statistically significant(P<0.05).Conclusion The level of the SCr,CysC and 24 hAPP are increased trend in elderly patients with hypertension with the increase age.The CysC and 24 hAPP are regarded as an evaluating indicator for early renal damage in elderly patients with hypertension.
