改良3钉固定与常规2钉固定治疗Tile B型骨盆骨折患者的效果对比观察

摘　　要：目的对比改良3钉固定与常规2钉固定治疗Tile B型骨盆骨折的临床效果。方法选取50例Tile B型骨盆骨折患者,随机分为两组各25例。A组给予改良3钉内固定治疗,B组给予常规2钉内固定治疗,比较两组患者术后2个月、 3个月及末次随访的Majeed评分、手术及恢复指标、并发症情况。结果术后2个月、 3个月,A组的Majeed评分显著高于B组(P <0.05),但末次随访两组的Majeed评分比较差异无统计学意义(P>0.05)。A组的手术时间显著长于B组,术中出血量显著高于B组,差异均有统计学意义(P <0.05)。两组的住院时间、并发症发生率比较无统计学差异(P>0.05)。结论改良3钉固定与常规2钉固定均可促进骨盆功能恢复,但改良3钉固定可减轻患者的早期耻骨疼痛,骨盆前环的稳定性更高。Objective To compare the clinical effects of modified 3 nail fixation and routine 2 nail fixation in the treatment of Tile B pelvic fracture. Methods 50 cases of patients with Tile B pelvic fracture were selected and randomly divided into two groups, with 25 cases in each group. Group A was treated with modified 3 internal fixation, and group B was treated with routine 2 nail internal fixation. The Majeed scores at 2 months and 3 months after operation and last follow-up, operation and recovery indicators, and complications were compared between the two groups. Results 2 months and 3 months after operation, the Majeed scores of group A were significantly higher than those of group B(P <0.05), but no statistical difference was found in the Majeed score between the two groups at last follow-up(P>0.05). The operation time of group A was significantly longer than that of group B, and the intraoperative blood loss was significantly higher than that of group B(both P <0.05). No statistical difference was found in the hospitalization time and incidence of complications between the two groups(P >0.05). Conclusions Modified 3 nail fixation and routine 2 nail fixation can promote the recovery of pelvic function, but modified 3 nail fixation can reduce the early pubic pain of patients, with higher stability of anterior pelvis.
