益肾降浊汤联合贝那鲁肽治疗糖尿病肾病的临床疗效及对患者血管内皮功能、肾功能的影响

摘　　要：目的观察益肾降浊汤联合贝那鲁肽治疗糖尿病肾病的临床疗效及对患者血管内皮功能、肾功能的影响。方法将105例糖尿病肾病患者按照随机数表法分为2组。对照组52例予贝那鲁肽治疗;观察组53例在对照组治疗基础上加用益肾降浊汤治疗。2组均治疗1个月后统计临床疗效,并观察2组治疗前后一氧化氮(NO)、内皮素1(ET-1)、24 h尿蛋白定量(24 hUTP)、血清肌酐(Cr)、血尿素氮(BUN)、空腹血糖(FPG)水平变化情况,以及中医症状积分和不良反应发生情况。结果观察组总有效率88.68%,对照组总有效率63.46%,观察组临床疗效优于对照组(P<0.05)。治疗后2组NO、ET-1水平均明显改善(P<0.05),且观察组NO水平高于对照组(P<0.05),ET-1水平低于对照组(P<0.05)。治疗后2组24 hUTP、Cr、BUN及FPG水平均下降(P<0.05),且观察组低于对照组(P<0.05)。治疗后2组尿浊、咽干口燥、神疲乏力及肢体水肿评分均明显下降(P<0.05),且观察组均低于对照组(P<0.05)。观察组不良反应总发生率5.66%,对照组不良反应总发生率28.85%,观察组低于对照组(P<0.05)。结论益肾降浊汤联合贝那鲁肽治疗糖尿病肾病疗效确切,并可改善患者血管内皮功能及肾功能。Objective To observe the efficacy of Yishen Jiangzhuo decoction combined with benalutide on diabetic nephropathy and its effect on vascular endothelial function and renal function. Methods 105 diabetic nephropathy patients were divided into two groups according to the random number table method. 52 cases in control group were treated by benalutide. 53 cases in observation group were treated by Yishen Jiangzhuo decoction on the basis of the control group. After one month of treatment, the clinical effects of the two groups were analyzed. The levels of nitric oxide(NO), endothelin 1(ET-1), 24 h urine protein(24 hUTP), serum creatinine(Cr), blood urea nitrogen(BUN), and fasting blood glucose(FPG), TCM symptom score and adverse reactions were observed before and after treatment between two groups. Results The total effective rate was 88.68% in observation group and 63.46% in control group. The clinical curative effect of the observation group was better than that of the control group(P<0.05). After treatment, the levels of NO and ET-1 in the two groups were significantly improved(P<0.05), and the level of NO in the observation group was higher than that in the control group(P<0.05), and the level of ET-1 was lower than that in the control group(P<0.05). After treatment, the levels of 24 hUTP, Cr, BUN and FPG in the two groups were decreased(P<0.05), and the observation group was lower than the control group(P<0.05). After treatment, the scores of urine turbidity, dry throat and dry mouth, fatigue and limb edema in the two groups were significantly decreased(P<0.05), and the observation group was lower than the control group(P<0.05). The total incidence of adverse reactions was 5.66% in observation group and 28.85% in control group, and the observation group was lower than the control group(P<0.05). Conclusion Yishen Jiangzhuo decoction combined with benaglutide in the treatment of diabetic nephropathy is effective and can improve vascular endothelial function and renal function.
