白癜风丸、308准分子光联合免疫制剂治疗白癜风稳定期临床疗效及对患者免疫的影响

摘　　要：目的观察白癜风丸、308准分子光联合免疫制剂治疗白癜风稳定期临床疗效及对患者免疫的影响。方法将90例白癜风稳定期患者按照随机数字表法分为2组。对照组45例予白癜风丸、308准分子光治疗;观察组45例在对照组治疗基础上加用免疫制剂治疗。2组均治疗3个月后统计临床疗效,观察2组治疗前后症状评分、外周血T淋巴细胞亚群、细胞炎症因子[干扰素γ(IFN-γ)、白细胞介素10(IL-10)]的变化和不良发应发生率。结果观察组总有效率86.67%,对照组总有效率68.89%,观察组临床疗效优于对照组(P<0.05)。治疗后2组心烦气躁、精神疲倦、白斑面积扩散、乳房胀痛评分及总分均降低(P<0.05),且观察组均低于对照组(P<0.05)。治疗后2组T淋巴细胞亚群CD3^+、CD4^+、CD4^+/CD8^+均升高(P<0.05),CD8^+均降低(P<0.05),且观察组CD3^+、CD4^+、CD4^+/CD8^+均高于对照组(P<0.05),CD8^+低于对照组(P<0.05)。治疗后2组血清IFN-γ水平均降低(P<0.05),且观察组低于对照组(P<0.05),治疗后观察组IL-10水平降低(P<0.05),且低于对照组治疗后(P<0.05),对照组治疗前后IL-10水平比较差异无统计学意义(P>0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论白癜风丸、308准分子光联合免疫制剂治疗白癜风稳定期疗效确切,能改善患者临床症状,调节免疫力。Objective To observe the efficacy of of vitiligo pill,308 excimer light combined with immune preparation in the treatment of vitiligo in stable stage and its effect on Tmmunity.Methods 90 patients with vitiligo in stable stage were divided into two groups according to random number table method.45 cases in control group were treated by vitiligo pill and 308 excimer light.45 cases in observation group were treated by immune preparation on the basis of the control group.After three months of treatment,the clinical effects of the two groups were analyzed.The symptom scores,peripheral blood T lymphocyte subsets,changes of cytokines[interferon-γ(IFN-γ),interleukin-10(IL-10)]and the incidence of adverse reactions were observed before and after treatment between two groups.Results The total effective rate was 86.67%in treatment group and 68.89%in control group.The clinical curative effect of the treatment group was better than that of the control group(P<0.05).After treatment,the scores of upset,mental fatigue,leukoplakia area spread,breast distention and pain,and total scores in the two groups were decreased(P<0.05),and the observation group was lower than the control group(P<0.05).After treatment,the T lymphocyte subsets CD3^+,CD4^+,CD4^+/CD8^+in the two groups were increased(P<0.05),CD8^+was decreased(P<0.05),and CD3^+,CD4^+,CD4^+/CD8^+in the observation group were higher than that in the control group(P<0.05),CD8^+was lower than that in the control group(P<0.05).After treatment,the level of IFN-γin the two groups was decreased(P<0.05),and the observation group was lower than the control group(P<0.05).After treatment,the level of IL-10 in the observation group was decreased(P<0.05),and the treatment groups was lower than that in the control group(P<0.05).There was no significant difference in the level of IL-10 before and after treatment in the control group(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The vitiligo pill,308 excimer ligh
