温和灸联合西药对急性高原肺水肿患者血红蛋白及动脉血气的影响

摘　　要：目的观察温和灸联合西药对急性高原肺水肿患者血红蛋白及动脉血气的影响。方法患者112例随机分为两组各56例。对照组采用常规西药治疗,在此基础上联合温和灸治疗作为联合组。观察两组患者疗效、血气及血流动力情况。结果联合组治疗总有效率为96.43%,明显高于对照组的83.93%(P<0.05)。治疗后联合组血红蛋白(Hb)以及动脉血二氧化碳分压(PaCO2)水平低于对照组(P<0.05),血氧饱和度(SaO2)以及氧合指数水平高于对照组(P<0.05)。治疗后联合组肺动脉平均压(mPAP)、肺动脉楔压(PAWP)、肺循环阻力指数(PVRI)以及右室每搏功指数(RVSWI)水平低于对照组,心输出指数(CI)以及每搏量(SVI)水平高于对照组(P<0.05)。治疗后联合组咳嗽、咯痰、胸闷气短以及口干咽燥评分均低于对照组(P<0.05)。结论温和灸联合常规西药对急性高原肺水肿患者具有良好效果,可明显改善患者动脉血气、血流动力学指标以及临床体征。Objective:To study the influence of mild moxibustion plus Western medicine on the hemoglobin and arterial blood gas of acute high-altitude pulmonary edema patients.Methods:112 patients were randomly assigned to two groups,56 cases in each.The control group took conventional Western medicine;the combined group took Western medicine plus moxibustion.The therapeutic effect,arterial blood gas and blood flow dynamics were explored.Results:The total effective rate in the combined group was significantly higher than the control group(P<0.05).Before treatment,there was no difference in all the indexes between groups(P>0.05).After treatment,the levels of Hb and PaCO2 were lower and the levels of SaO2 and oxygenation index in the combined group were higher than those in the control group(P<0.05).The levels of mPAP,PAWP,PVRI and RVSWI were lower,while the levels of CI and SVI in the combined group were higher than those in the control group(P<0.05).Conclusion:Mild moxibustion combined with conventional Western medicine has a good effect on patients with acute high altitude pulmonary edema,significantly improving the arterial blood gas,hemodynamics and clinical signs.
