中医药镇痛方案在人工膝关节置换术围手术期的疗效观察

摘　　要：目的:研究中医药镇痛方案在人工膝关节置换术(Total Knee Arthroplasty,TKA)围手术期的效果。方法:抽取我院2017年1月-2019年1月行人工膝关节置换术患者58例,根据镇痛方式的不同分为两组均29例,其中对照组采取常规西药治疗,研究组在此基础上实施中医镇痛方案,对比两组患者疼痛、膝关节各项评分情况。结果:两组患者在手术前1天疼痛之间无差异,术前两组患者膝关节评分系统(Hospital for Special Surgery Knee Score,HSS)、Berg平衡量表(Berg Balance Scale,BBS)评分无差异,术前3天与术后7天两组患者膝关节活动度(Range Of Motion,ROM)值无明显差异,P>0.05;研究组在术后3天、10天、20天视觉模拟评分法(Visual Analogue Scale,VAS)均低于对照组,术后30天研究组HSS、BBS评分均高于对照组,术后1天、14天、30天上研究组ROM值均优于对照组,差异显著,P<0.05。结论:对于人工膝关节置管术患者,可采取常规西药联合中医药镇痛方案,有效改善患者手术后的疼痛情况,并有利于患者尽早进行功能锻炼,及时恢复期关节功能,值得应用。Objective:To study the effect of analgesia with TCM medicine during perioperative period of TKA.Methods:58 patients from January 2017 to January 2019 in our hospital were divided into two groups according to different analgesia methods,29 cases in each.The control group was treated with conventional western medicine.The study group was treated with TCM medicine more.The scores of pain and knee were compared in the two groups.Results:There was no difference in the scores of the pain and knee,HSS and BBS between two groups at the day before operation.There was no significant difference in ROM between the two groups 3 days before the operation and 7 days after the operation,P>0.05.VAS in the study group was lower than the control group in 3 days,10 days,20 days after the operation.HSS,BBS in the study group were higher than the control group in 30 days after the operation.ROM in the study group was more improved than the control group in one days,14 days,30 days after the operation,P<0.05.Conclusion:Conventional western medicine plus analgesic with TCM can improve the postoperative pain and is conducive to early functional exercise and timely recovery of joint function for patients with artificial knee catheterization.
