摘　　要：目的探讨助产士参与产前门诊对初产妇分娩结局的影响。方法选择2018年2—8月在本院产科建档并计划在本院分娩的100例孕产妇作为研究对象,根据随机数表法分为观察组和对照组两组,每组各50例。对照组接受常规的产前门诊,观察组则采用助产士参与产前门诊的模式进行干预。比较两组的产前焦虑程度(SAI评分)、妊娠结局及满意度。结果观察组孕产妇分娩前SAI评分为(39.16±2.08)分,明显低于对照组的(43.28±3.15)分,差异有统计学意义(P<0.05);观察组剖宫产率为22.00%,明显低于对照组的40.00%,差异有统计学意义(P<0.05);两组孕妇的总产程、产后2 h出血量以及新生儿体重、新生儿Apgar评分比较,差异均无统计学意义(P>0.05);观察组孕妇满意度明显高于对照组(94.00%VS 82.00%),差异有统计学意义(P<0.05)。结论助产士参与产前门诊具有较好的应用价值,可以有效减轻孕产妇的心理压力,降低剖宫产率,提高围产期满意度。Objective To explore the effect of midwives'participation in prenatal clinic on the delivery outcome of primipara.Methods 100 cases of pregnant women who were filed in obstetrics department of our hospital from February 2018 to August 2018 and planned to give birth in our hospital were selected as the research objects.According to the random number table method,they were divided into two groups,50 cases in each group.The control group received routine prenatal clinic,while the observation group used the mode of midwives participating in prenatal clinic to intervene.The degree of prenatal anxiety(SAI score),pregnancy outcome and satisfaction were compared between the two groups.Results The SAI score of pregnant women in the observation group before delivery was(39.16±2.08),which was significantly lower than(43.28±3.15)of the control group,the difference was statistically significant(P<0.05).The cesarean section rate in the observation group was 22.00%,which was significantly lower than 40.00%of the control group(P<0.05).The total course of labor,the amount of postpartum hemorrhage in 2 hours,the weight of newborn and the Apgar score ratio of newborn were not significantly different between the two groups(P>0.05).The satisfaction rate of pregnant women in the observation group was significantly higher than that in the control group(94.00%VS 82.00%,P<0.05).Conclusions Midwives'participation in prenatal clinic performs well application value,could effectively reduce the psychological pressure of pregnant women,reduce the rate of cesarean section,and improve perinatal satisfaction.
