摘 要:目的评价锥形束CT(cone-beam computed tomography,CBCT)和数字化牙片(digital periapical radiographs,DPRs)测量牙齿长度的准确性。方法需拔牙的正畸患者拍摄CBCT,共46例患者82颗前磨牙纳入研究。利用CBCT自带软件从轴位(axial planes,APs)、矢状位(sagittal planes,SPs)和冠状位(coronal planes,CPs)测量牙齿的长度。牙齿拔出后,利用游标卡尺测量牙齿长度作为金标准。然后将牙齿放入人干头颅的相应牙槽窝内,使用分角线技术拍摄DPRs,并测量牙齿长度。比较CBCT和DPRs测量牙齿长度的准确性。结果单根前磨牙,CBCT-轴位、矢状位和冠状位的测量值均低于牙齿实际长度,但差异无统计学意义(P>0.05),而数字化牙片测量值显著高于牙齿实际长度(P<0.05)。双根前磨牙,CBCT-轴位测量值高于牙齿实际长度,CBCT-矢状位和冠状位测量值低于牙齿实际长度,但差异均无统计学意义(P>0.05)。数字化牙片测量值显著低于颊根的实际长度,高于腭根的实际长度(P<0.05)。结论CBCT测量牙齿长度显示较高的可信度。Objective To evaluate the accuracy of cone-beam computed tomography(CBCT)and digital periapical radiographs(DPRs)for tooth length measurements.Methods Forty-six CBCT images and 82 premolars were collected from the patients who were going to undergo orthodontic treatment and would require premolar extractions.The tooth length measurements made by digital caliper were considered as the gold standard.The tooth length was measured by CBCT software on axial planes(APs),sagittal planes(SPs)and coronal planes(CPs).Then the extracted teeth were put into the corresponding location of a dry skull.Standardized bisecting angle DPRs were taken and tooth length was measured.Accuracy of tooth length measured by CBCT and DPRs were compared based on the gold standard.Results For single-rooted premolars,the CBCT measurements underestimated the tooth length at every plane with no statistical significance(P>0.05),while DPRs measurements overestimated the tooth length significantly(P<0.05).For two-rooted premolars,the CBCT-APs measurements overestimated the tooth length,but the CBCT-SPs and CBCT-CPs underestimated the tooth length.However,these differences had no statistical significance(P>0.05).The DPRs measurements underestimated the buccal length but overestimated the palatal length significantly(P<0.05).Conclusion CBCT tooth length measurements show a strong correlation with the gold standard at every plane,which indicates higher reliability.
