补肾活血法治疗高血压的Meta分析

摘　　要：目的:对补肾活血法治疗原发性高血压(essential hypertension,EH)进行系统评价。方法:检索中国知网、万方数据库、维普网、中国生物医学文献数据库、PubMed、Cochrane Library数据库,搜索“补肾活血法”治疗EH的随机对照试验,使用Cochrane评价手册5.1.0对纳入文献进行质量评估,并应用Revman 5.3进行数据分析。结果:纳入18篇文献,共1606例EH患者,Meta分析提示:与单纯西药比较,补肾活血类中药能有效提高EH的降压疗效[OR=2.73,95%CI(1.91,3.89)],降低患者收缩压[SMD=-7.78,95%CI(-10.06,-5.51)]及舒张压[SMD=-3.58,95%CI(-6.36,-0.80)],改善患者的临床症状[OR=4.66,95%CI(3.38,6.41)],且安全性较好。结论:补肾活血法治疗EH,可提高降压疗效,改善患者的临床症状,且安全性较好,但由于纳入文献质量偏低,还需更多高质量的随机对照试验进一步验证。Objective:To systematically evaluate the therapeutic effect of method of invigorating kidney and promoting blood circulation on essential hypertension(EH).Methods:CNKI,Wanfang database,VIP,China biomedical literature database,PubMed,and Cochrane library database were used for retrieval;the randomized controlled trial of“method of invigorating kidney and promoting blood circulation”for EH was searched.Cochrane evaluation manual 5.1.0 was used to evaluate the quality of the included literature,and Revman 5.3 was used for data analysis.Results:A total of 1606 EH patients were included in 18 literatures.Meta analysis showed that compared with pure Western medicine,Chinese medicinals of invigorating kidney and promoting blood circulation could effectively improve the antihypertensive effect of EH[OR=2.73,95%CI(1.91,3.89)],decreased systolic blood pressure[SMD=-7.78,95%CI(-10.06,-5.51)]and diastolic pressure[SMD=-3.58,95%CI(-6.36,-0.80)],improve the clinical symptoms of patients[OR=4.66,95%CI(3.38,6.41)],and the safety was good.Conclusion:The method of invigorating the kidney and promoting blood circulation can improve the effect of blood pressure reduction,enhance the clinical symptoms of patients,and has a good safety.However,due to the low quality of the included literature,more high-quality randomized controlled trials are needed to further verify.
