基于数据挖掘分析穴位敷贴治疗功能性消化不良选穴与用药规律

摘　　要：目的:基于数据挖掘分析穴位敷贴治疗功能性消化不良(Functional dyspepsia,FD)选穴与用药规律。方法:检索万方数据库、维普中文科技期刊、中国期刊全文数据库(CNKI)、中国生物医学(CBM)四大数据库。检索时间为各数据库建库至2018年12月30日。以“穴位贴敷”“穴位敷贴”“穴位外敷”“腧穴敷贴”“功能性消化不良”为检索词,采用主题词和自由词相结合的方式进行检索。结果:对穴位贴敷治疗FD的临床文献进行检索及整理,共检出252篇文章,符合纳入标准文献48篇,其中穴位21个,中药72味。使用频次在5次以上的腧穴有10个,使用频次前3位的分别为中脘、神阙、天枢穴。穴位贴敷治疗FD的腧穴共涉及6条经脉,其中以任脉、督脉最多。使用频次在5次以上的药物有26味,使用频次较多的有细辛、白芥子、吴茱萸、肉桂、木香等。穴位贴敷治疗FD所用中药使用频次在10次以上的有7味,以温里药、理气药为主。穴位贴敷主治FD的用药归经以脾、胃、肾经为主。结论:针灸治疗FD选穴以脘腹部的任脉穴位为主,此外督脉、足阳明胃经穴位也较为常用,而用药则以具有温阳、理气、散寒之功的辛温药物居多。Objective:To analyze point selection and medication law of acupoint application for functional dyspepsia(FD)based on data mining.Methods:Wanfang database,VIP Chinese sci-tech periodicals,Chinese journal full text database(CNKI),and Chinese Biomedical(CBM)were searched.The retrieval time was the establishment of each database until December 30,2018."Acupoint application","acupoint external application",and"functional dyspepsia"and so on were taken as search terms,the retrieval was carried out by combining subject words and free words.Results:Retrieving and sorting out the clinical literature of acupoint application for FD,252 articles were found,48 of which met the inclusion criteria,including 21 acupoints and 72 traditional Chinese medicines.Ten acupoints were used more than five times,and the first three were Zhongwan(RN 12),Shenque(RN 8),and Tianshu(ST 25).There were 6 meridians involved in acupoint application for FD,of which conception vessel(CV)and governor vessel(GV)are the most frequently used.Twenty-six medicines were used more than five times,including Xixin(Radix et Rhizoma Asari),Baijiezi(Semen Sinapis),Wuzhuyu(Fructus Evodiae),Rougui(Cortex Cinnamomi),Muxiang(Radix Aucklandiae),etc.Acupoint application of TCM for FD has been used more than 10 times in 7 flavors,mainly for warming the interior and regulating qi.Acupoint application for FD is mainly attributed to the spleen,stomach and kidney meridians.Conclusion:Acupuncture and moxibustion are mainly used at CV acupoints in abdomen.In addition,GV acupoints and Foot Yangming stomach meridians acupoints are also commonly used.The most commonly used drugs are pungent-warm medicinals with the functions of warming yang,regulating qi and dispelling cold.
