股前外侧皮瓣治疗糖尿病足的临床疗效

摘　　要：目的探讨采用股前外侧皮瓣修复糖尿病足创面的临床效果。方法自2013年6月至2018年1月,对32例糖尿病足创面溃疡及感染的患者行股前外侧皮瓣修复术。术前严格控制血糖,并予以抗感染及清创治疗,对较严重的感染创面联合应用VSD负压吸引技术控制感染;皮瓣供区创面直接拉拢缝合。结果本组共32例患者,术后皮瓣血运良好,成活率为93.75%(30/32),2例皮瓣远端发生部分坏死,经换药后创面愈合。随访6~18个月,所有患者的脚踝功能及行走功能均恢复。结论采用股前外侧皮瓣修复糖尿病足溃疡及感染创面,效果较明显,且创面可一期愈合;部分患者修复部位的皮肤感觉得到了不同程度的恢复。Objective To explore the clinical efficacy of anterolateral thigh flap in the repair of diabetic foot wounds.Methods From June 2013 to January 2018,32 patients with diabetic foot ulcer and infection were treated with anterolateral thigh flaps.Preoperative blood sugar was strictly controlled,anti-infection and debridement were given.VSD negative pressure suction was used to control infection in severe infected wounds.The donor site of skin flaps was directly closed and sutured.Results A total of 32 patients received the treatment.The blood supply of flaps was good after operation.The survival rate was 93.75%(30/32).Two patients had partial necrosis at the distal end of the flaps and the wound healed after dressing change.All patients were followed-up from 6 to 18 months,the function of ankle and walk of all patients were recovered.Conclusion The anterolateral thigh flap was effective in the repair of diabetic foot ulcer and infected wound,and the wound can heal in one stage.Skin sensation of repaired parts of some patients has been restored to varying degrees.
