摘 要:目的研究枸橼酸咖啡因对极低体质量早产儿呼吸暂停的疗效及安全性分析。方法选取2015年1月至2017年7月郑州市第一人民医院收治的39例呼吸暂停患儿,治疗组20例,对照组19例。对照组予以氨茶碱治疗,治疗组予以枸橼酸咖啡因治疗。对比两组治疗效果和副反应等。结果治疗后,治疗组患儿的疗效为95.00%(19/20),高于对照组临床疗效78.95%(15/19),差异有统计学意义(P<0.05);颅内出血(5.00%vs 31.58%)、支气管肺发育不良(10.00%vs 36.84%)及视网膜病变(10.00%vs 26.32%)等并发症发生率低于对照组,差异均有统计学意义(P<0.05);观察组脑室周围白质软化(0.00%vs 10.53%)的发生率低于对照组,但差异无统计学意义(P>0.05)。对照组不良反应总发生率为15.00%,与对照组57.89%相比明显降低,差异有统计学意义(P<0.05)。随访至纠正胎龄40周,治疗组NBNA评分结果明显高于对照组(P<0.05)。结论对极低体质量早产儿呼吸暂停,予以枸橼酸咖啡因治疗可显著缩短患儿的药物显效时间,降低患儿的支气管肺发育不良率,促进患儿的脑神经发育,减少治疗过程中的不良反应。Objective To study the effect and safety of caffeine citrate on apnea in very low birth weight premature infants.Methods 40 children with apnea admitted to Zhengzhou First People s Hospital from January 2015 to July 2017 were randomly divided into two groups,20 cases in the treatment group,and 19 in the control group.The control group was treated with aminophylline,and the treatment group was treated with caffeine citrate.The therapeutic effect and side effect of the two groups were compared.Results After treatment,the curative effect of the treatment group was 95.00%(19/20),which was higher than that of the control group(78.95%(15/19),P<0.05).The incidence of intracranial hemorrhage(5.00%vs 31.58%),bronchopulmonary dysplasia(10.00%vs 36.84%)and retinopathy(10.00%vs 26.32%)was lower in the treatment group than that of the control group,with significant difference(P<0.05).The incidence of periventricular leukomalacia(0.00%vs 10.53%)in the treatment group was lower than that in the control group,without significant difference(P>0.05).The general occurrence rate of the treatment group was 15.00%,which was lower than that of the control group(57.89%),with significant difference(P<0.05).After 40 weeks of follow-up,NBNA score was significantly higher in the treatment group than in the control group(P<0.05).Conclusion Caffeine citrate treatment for very low birth weight premature infants with apnea can significantly shorten the effective time of drugs,reduce the incidence of bronchopulmonary dysplasia,promote the development of brain nerve and reduce adverse reactions during treatment.
