猪肺磷脂注射液配合常规疗法对呼吸窘迫新生儿支气管肺发育不良及PLTP、MIF的影响

摘　　要：目的探讨猪肺磷脂注射液配合常规机械通气对呼吸窘迫新生儿支气管肺发育不良的效果及对PLTP和MIF的影响。方法选择2017年1月至2019年1月来我院就诊的呼吸窘迫综合征患儿80例,随机分为治疗组和对照组,每组40例患者;对照组患儿给予猪肺磷脂注射液,100~200mg/kg,如果治疗后12h病情无明显好转,可按初始剂量再给药一次,治疗组患儿在对照组基础上联合常规机械通气治疗。观察治疗前及治疗后,血气及呼吸机参数变化、血清PLTP、MIF水平的变化。结果治疗组PaO2较治疗前升高,治疗组高于对照组,PaCO2较治疗前降低,治疗组低于对照组(P<0.05),两组PH值均较治疗前升高,治疗组较对照组无明显差异(P>0.05);治疗后两组患者主要呼吸机参数PIP、PEEP、FiO2均降低,治疗组明显低于对照组(P<0.05);治疗后治疗组MIF水平明显低于对照组,治疗组PLTP水平升高且明显高于对照组(P<0.05)。治疗后对照组25例患儿肺野透亮度改善,颗粒网状阴影消失,治疗组33例患儿肺野透亮度改善,颗粒网状阴影消失。结论猪肺磷脂注射液配合常规机械通气治疗能增加血清中PLTP水平,降低MIF水平,改善呼吸窘迫新生儿氧合功能,减少支气管发育不良的发生,值得在临床推广。Objective To explore the effect of porcine lung phospholipid injection combined with conventional mechanical ventilation on bronchopulmonary dysplasia in neonates with respiratory distress and the effect on PLTP and MIF.Methods 80 children with respiratory distress syndrome treated in our hospital from January 2017 to January 2019 were randomly divided into the treatment group and the control group,with 40 patients in each group.The control group was given pig lung phospholipid injection,100-200 mg/kg.If the condition did not improve significantly after 12 hours of treatment,the children in the treatment group could be given another routine treatment and mechanical ventilation according to the initial dosage.The patients in the treatment group were combined on the basis of the control group.In combination with conventional mechanical ventilation,100-200 mg/kg phosphatidylcholine injection could be given again according to the initial dosage if the disease did not improve significantly 12 hours after treatment.Changes of blood gas and ventilator parameters,serum PLTP and MIF levels before and after treatment were observed.Results The level of PaO2 increased after treatment,and it was higher in the treatment group than in the control group.The level of PaCO2 decreased after treatment,and it was higher in the treatment group than in the control group(P<0.05).The PH value in both groups were higher than those before treatment,and there was no significant difference between the two groups(P>0.05).PIP,PEEP and FiO2 in the two groups were lower after treatment,and MIF level in the treatment group was lower than that in the control group(P<0.05).MIF level in the treatment group was lower than that in the control group at all time points.The level increased gradually and was higher than that of the control group at all time points(P<0.05).After treatment,the transparency of lung field was improved in 25 children in the control group,and the granular reticular shadow disappeared.In the treatment group,the transparency of l
