摘 要:目的对急性肾盂肾炎患者的病原菌进行分析,探讨其感染病原菌的分布特点。方法100例急性肾盂肾炎患者,根据发作次数分为首发组(初次发作,62例)和复发组(多次发作,38例)。取两组患者清中段尿实施病原菌培养和药敏试验。比较两组患者病原菌阳性率、病原菌分布情况。结果复发组病原菌阳性率为78.95%(30/38),高于首发组的53.23%(33/62),差异有统计学意义(P<0.05)。首发组检出33株病原菌,其中革兰阴性菌23株,包括大肠埃希菌16株,不动杆菌属3株,变形杆菌属2株,克雷伯菌2株;革兰阳性菌10株,其中肠球菌属5株,金黄色葡萄球菌2株,凝固酶阴性葡萄球菌3株。复发组检出30株病原菌,其中革兰阴性菌17株,包括大肠埃希菌13株,不动杆菌属2株,变形杆菌属1株,克雷伯菌1株;革兰阳性菌13株,其中肠球菌属9株,金黄色葡萄球菌2株,凝固酶阴性葡萄球菌2株。两组患者病原菌分布情况比较,差异无统计学意义(χ^2=1.151,P>0.05)。结论急性肾盂肾炎患者感染病原菌主要为大肠埃希菌和肠球菌属,病情发作的次数对感染病原菌的构成不具影响。Objective To analyze the pathogens in patients with acute pyelonephritis,and discuss the distribution characteristics of the pathogenic bacteria.Methods A total of 100 patients with acute pyelonephritis were divided by frequency of attacks into initial group(initial attack,62 cases)and recurrent group(multiple attacks,38 cases).The pathogenic bacteria culture and drug susceptibility test were performed in the middle urine of the two groups.The positive rate and distribution of pathogenic bacteria was compared between the two groups.Results The positive rate of pathogenic bacteria was 78.95%(30/38)in recurrent group,which was higher than 53.23%(33/62)in initial group,and the difference was statistically significant(P<0.05).In the initial group,33 strains of pathogenic bacteria were detected,of which 23 strains were Gram-negative bacteria,including 16 strains of Escherichia coli,3 strains of Acinetobacter,2 strains of Proteus and 2 strains of Klebsiella;10 strains of Gram-positive bacteria,including 5 strains of Enterococcus,2 strains of Staphylococcus aureus and 3 strains of coagulase negative Staphylococcus.In the recurrence group,30 strains of pathogenic bacteria were detected,of which 17 strains were Gram-negative bacteria,including 13 strains of Escherichia coli,2 strains of Acinetobacter,1 strain of Proteus and 1 strain of Klebsiella;13 strains of Gram-positive bacteria,including 9 strains of Enterococcus,2 strains of Staphylococcus aureus and 2 strains of coagulase negative Staphylococcus.There was no statistically significant difference in distribution of pathogenic bacteria between the two groups(χ^2=1.151,P>0.05).Conclusion The main pathogens of acute pyelonephritis were Escherichia coli and Enterococcus,and the frequency of attack has no effect on the composition of the pathogens.
