生肌愈疡膏联合负压封闭引流治疗蛇伤溃疡的临床研究

摘　　要：目的观察生肌愈疡膏联合负压封闭引流(VSD)治疗蛇伤溃疡热毒蕴结证的疗效。方法选择已经确诊的蛇伤溃疡热毒蕴结证患者60例,随机分为2组,观察组采用外敷生肌愈疡膏联合负压封闭引流,对照组采用外敷美宝湿润烧伤膏联合负压封闭引流,观察2组治疗前后中医证候积分变化和临床疗效,疮面恢复情况(新鲜肉芽覆盖时间、住院时间、换药次数),创面碱性成纤维细胞生长因子(bFGF)和表皮细胞生长因子(EGF)水平的变化。结果治疗后2组中医证候积分均下降(P<0.05~0.01),观察组优于对照组(P<0.05);总有效率观察组优于对照组(P<0.05);观察组比对照组的肉芽覆盖时间、住院时间短,换药次数少(P<0.05);2组创面bFGF和EGF水平明显增高(P<0.05~0.01),观察组优于对照组(P<0.05)。结论生肌愈疡膏联合负压封闭引流(VSD)治疗蛇伤溃疡疗效佳、恢复时间短及预后较好。OBJECTIVE To observe the efficacy of Shenji Yuyang ointment combined with vacuum sealing drainage(VSD) on treating snakebite ulcer.METHODS 60 patients diagnosed as having snakebite ulcer has been randomly selected into two groups. The observation group was given Shenji Yuyang ointment combined with VSD while the control group was given MEBO’s scald-plasters and VSD. Their traditional Chinese medicine(TCM) scores, efficacy, wound recovery(the duration of granulation covering wounds, hospital stays, dressing changing times), basic fibroblast growth factor(bFGF), and epidermal growth factor(EGF) before and after treatment had been observed. RESULTS After treatment, TCM scores of two groups have decreased(P<0.05, P<0.01) and the observation group was more significant. In terms of total effective rate, the observation group was more superior. The observation group had less duration of granulation covering wounds, hospital stays and dressing changing times(P<0.05). The level of two groups’ bFGF and EGF has significantly increased(P<0.05, P<0.01), among which the observation group was better(P<0.05). CONCLUSION Shenji Yuyang ointment combined with VSD has better effect on treating snakebite ulcer with less recovery time and better prognosis.
