狼疮静颗粒联合西药治疗系统性红斑狼疮肾虚瘀毒证临床研究

摘　　要：目的探讨狼疮静颗粒联合西药治疗系统性红斑狼疮(SLE)肾虚瘀毒证的有效性与安全性。方法选取医院2016年1月至2018年3月收治的SLE患者40例,随机均分为对照组和观察组,各20例。对照组患者给予泼尼松+硫酸羟氯喹治疗,观察组患者在对照组患者治疗基础上联用狼疮静颗粒,两组均持续用药12周。结果观察组的临床总有效率为95.00%,显著高于对照组的70.00%(P<0.05);观察组不良反应发生率为10.00%,低于对照组的40.00%(P<0.05);治疗第6周、治疗结束时,两组中医证候积分、SLE疾病活动度(SLEDAI)评分均较入院时下降,且观察组下降程度大于对照组(P<0.05);治疗结束时,两组患者左室内径(LV)、左房内径(LA)、室间壁厚度(IVS)、左室后壁厚度(PW)检测水平均较入院时降低,且观察组降幅大于对照组(P<0.05)。结论狼疮静颗粒联合西药治疗SLE疗效确切,患者临床症状及体征缓解更好、更快,心脏损害明显减轻,可最大限度地减少因激素治疗而引起的不良反应,增效解毒之功理想。Objective To investigate the efficacy and safety of Langchuangjing Granules combined with Western medicine in the treatment of systemic lupus erythematosus(SLE) with the syndrome of kidney deficiency and toxin stasis.Methods Totally 40 patients with SLE admitted to our hospital from January 2016 to March 2018 were selected and randomly divided into the control group and the observation group by 1∶1 method,20 cases in each group.The patients in the control group were treated with prednisone and hydroxychloroquine sulfate,and the patients in the observation group were treated with Langchuangjing Granules on the basis of the control group.Both groups were continuously treated for 12 weeks.Results The total clinical effective rate of the observation group was95.00%,which was significantly higher than 70.00% of the control group(P<0.05).The incidence of adverse reactions in the observation group was 10.00%,which was lower than 40.00% in the control group(P<0.05).After 6 weeks of treatment and at the end of treatment,the scores of traditional Chinese medicine(TCM) syndrome and systemic lupus erythematosus disease activity index(SLEDAI) were lower than those at admission,the decrease in the observation group was more significant than that in the control group(P<0.05).At the end of treatment,the levels of left ventricular diameter(LV),left atrial diameter(LA),interventricular septal thickness(IVS),left ventricular posterior wall thickness(PW) in the two groups were lower than those at admission,and the decrease in the observation group was more significant than that in the control group(P<0.05).Conclusion Langchuangjing Granules combined with Western medicine are effective in the treatment of SLE,which can relieve clinical symptoms and signs,alleviate cardiac damage,minimize adverse reactions caused by hormone therapy,and achieve the satisfactory effect of detoxification.
