清热解毒中药纱条填塞对低位肛周脓肿术后疼痛及创面愈合的影响

摘　　要：目的观察清热解毒中药纱条填塞与甲硝唑纱条填塞对低位肛周脓肿术后疼痛及创面愈合的影响。方法将300例低位肛周脓肿患者随机分为2组,2组均采用一次性肛周脓肿根治术,对照组术后给予甲硝唑纱条填塞换药治疗,观察组术后给予清热解毒中药纱条填塞换药治疗,连续治疗21 d,对比2组临床疗效、临床康复指标(腐肉脱落时间、创面干燥时间、创面愈合时间)、疼痛视觉模拟评分法(VAS)评分和不良反应发生情况。结果观察组痊愈率为46.0%(69/150),对照组为27.3%(41/150),观察组痊愈率明显高于对照组(χ^2=10.345,P<0.05);观察组创面腐肉脱落时间、创面干燥时间、创面愈合时间均明显短于对照组(P均<0.05);2组术后7 d VAS评分明显升高,术后14,21 d均呈明显降低趋势,且观察组术后7 d、14 d、21 d的VAS评分均明显低于同期对照组(P均<0.05);2组均未发生明显不良反应。结论清热解毒中药纱条填塞能够缓解低位肛周脓肿术后患者创面渗出和创缘水肿,有助于缓解创面疼痛,促进创面愈合。Objective It is to observe the effect of gauze packing with traditional Chinese medicine for clearing heat and removing toxin or metronidazole on postoperative pain and wound healing in low perianal abscess.Methods 300 patients with low perianal abscess were randomly divided into 2 groups.Both groups were treated with one-time radical resection of perianal abscess.The control group was treated with metronidazole gauze by packing and changing dressing.The observation group was treated with traditional Chinese medicine for clearing heat and removing toxin by packing and changing dressing.Both groups were treated for 21 days.The clinical efficacy,clinical rehabilitation indexes(carrion shedding time,wound drying time,wound healing time),pain visual analogue scale(VAS)scores and adverse reaction were compared between the two groups.Results The cure rate of the observation group was 46.0%(69/150),and that of the control group was 27.3%(41/150),the cure rate of the observation group was higher than that of the control group(χ^2=10.345,P<0.05).The carrion shedding time,wound drying time,wound healing time were significantly shorter in the observation group than those in the control group(P<0.05).At 7 days after operation,the VAS scores of the two groups were significantly increased,and then decreased significantly(P<0.05).The VAS of the observation group at 7d,14d and 21d after operation were significantly lower than those in the control group(P<0.05);no significant adverse reactions occurred in either group.Conclusion Gauze packing with traditional Chinese medicine for clearing heat and removing toxin can alleviate wound exudation and edema,relieve wound pain and promote wound healing in the patients after one-time radical resection of perianal abscess.
