摘  要:目的通过Hisense计算机辅助手术系统(Hisense CAS)对重建的肝脏门静脉主干结构进行分析,为肝脏手术的个体化精准治疗提供理论依据。方法应用Hisense CAS对965例符合纳入标准的不同年龄段人群正常上腹增强CT检查影像资料的DICOM文件进行肝脏及其血管的三维重建,对门静脉主干的分型及变异进行数据分析。结果Hisense CAS可以精确重建出肝脏及门静脉四级分支,清晰展示血管走行。965例中门静脉主干分为6型:Ⅰ型806例(83.52%),Ⅱ型70例(7.25%),Ⅲ型8例(0.83%),Ⅳ型74例(7.67%),Ⅴ型6例(0.62%),Ⅵ型1例(0.10%)。结论门静脉主干分型变异较多,Hisense CAS能清晰显示门静脉血管的解剖变异及走行方向,帮助术者术前准确了解个体的门静脉主干分型,以利于肝脏手术的精准实施。Objective To investigate the structure of the reconstructed main portal vein using the Hisense computer-assisted surgery system(CAS),and to provide a theoretical basis for individualized precise treatment with liver surgery.Methods Hisense CAS was used to perform three-dimensional reconstruction of the liver and its vessels based on the original normal contrast-enhanced CT DICOM files(upper abdomen)of 965 subjects with different ages who met the inclusion criteria.A data analysis was performed for the classification and variation of the main portal vein.Results Hisense CAS accurately reconstructed the li-ver and the portal vein branches up to level 4 and clearly showed the shape of the vessels.Among the 965 patients,806(83.52%)had typeⅠ,70(7.25%)had typeⅡ,8(0.83%)had typeⅢ,74(7.67%)had typeⅣ,6(0.62%)had typeⅤ,and 1(0.10%)had typeⅥmain portal vein.Conclusion The main portal vein has various types and variations.Hisense CAS can clearly show the anatomical variation and shape of the portal vein.Accurate evaluation of individual blood vessel classification before surgery may help to perform a precise liver surgery.
