摘  要:目的探讨亚甲基四氢叶酸还原酶(MTHFR)(C677T)基因T型变异升高同型半胱氨酸(Hcy)对汉族人群冠状动脉狭窄程度的影响。方法收集2016年9月至2018年1月至该院行冠状动脉造影的汉族患者700例,分为冠心病组(456例)和健康对照组(244例)。检测其血液MTHFR基因第677位CC、TC、TT的多态性、血清Hcy水平,依据冠状动脉造影结果将冠状动脉病变程度分为A、B和C级,统计分析其数据的相关性。结果各基因型患者性别、年龄比较,差异无统计学意义(P>0.05);TT、TC和CC型患者血清Hcy水平分别为(20.17±8.87)、(14.62±9.55)、(12.74±6.68)μmol/L,差异有统计学意义(P<0.05);A级患者血清Hcy水平为(13.48±7.69)μmol/L,B级(16.79±8.06)μmol/L,C级(19.84±7.97)μmol/L,各级患者血清Hcy水平比较差异有统计学意义(P<0.05);Logistic回归显示,Hcy水平与冠心病发生相关,但差异无统计学意义(OR=1.079;P=0.140);冠状动脉病变最严重为TT基因型,各组冠状动脉病变程度差异有统计学意义(P<0.05);MTHFR(C677T)基因TT型和TC型与冠心病发生相关,但差异无统计学意义(P>0.05)。结论在湖州市汉族人群中,血清Hcy水平及MTHFR(C677T)基因变异与冠状动脉狭窄程度相关,其中高水平Hcy及MTHFR(C677T)TT与TC基因型均可能是冠心病发生的危险因素。Objective To investigate the effect of T-type variation of MTHFR(C677T)gene for increasing homocycteine(Hcy)on the degree of coronary stenosis in Han population.Methods Seven hundreds patients with coronarial angiography in the hospital from September 2016 to January 2018 were collected and divided into the coronary heart disease group(n=456)and healthy control group(n=244).The CC,CT and TT polymorphisms at blood MTHFR gene 677 locus and serum Hcy level were detected.According to the results of coronarial angiography,the degrees of coronary arterial disease were divided into the level A,B and C.The correlation among the data was statistically analyzed.Results There was no statistical difference in sex and age among the patients with various genotypes.The serum Hcy levels of TT,CT and CC genotypes were(20.17±8.87)μmol/L and(14.62±9.55)μmol and(12.74±6.68)μmol/L respectively;the serum Hcy level in the patients with grade A was(13.48±7.69)μmol/L,which in the patients with grade B was(16.79±8.06)μmol/L and which in the grade C was(19.84±7.97)μmol/L,there were statistically significant differences among the three various grades(P<0.05);the Logistic regression showed that Hcy was correlated with the occurrence of coronary heart disease,but the difference had no statistical significance(OR=1.035,P=0.140);The most serious of coronary arterial lesion was the TT genotype,the severity degree of coronary arterial lesions had statistical difference among various groups(P<0.05).The genotype TT and TC of MTHFR(C677T)gene were correlated with the occurrence of coronary heart disease,but the difference had no statistical significance(P>0.05).Conclusion The serum Hcy level and MTHFRC677T gene variation are correlated with the stenosis degree of coronary artery in the Han population in Huzhou City,serum Hcy level and MTHFRC677T gene variation have the correlation with the degree of coronary arterial stenosis,in which high level Hey and the genotype TT and TC of MTHFR(C677T)all may be the risk factors of coronary heart d
