口服二甲双胍对2型糖尿病患者血清维生素B12水平的影响

摘　　要：目的:探讨口服二甲双胍对2型糖尿病患者血清维生素B12水平的影响。方法:收集2015年8月至2018年12月中日友好医院(以下简称“我院”)维生素血药浓度监测患者的病历资料进行回顾性分析,根据患者用药情况分为二甲双胍组与对照组,比较两组患者血清维生素B12水平;以血清维生素B12<200 pg/ml为界限判定为缺乏,比较两组患者维生素B 12缺乏情况;根据二甲双胍剂量分为低剂量组、中剂量组和高剂量组,根据二甲双胍用药时间分为1年、1~2年和3年及以上组,分别比较血清维生素B12水平的差异。结果:共纳入二甲双胍组患者134例、对照组患者379例,两组患者血清维生素B12水平的差异有统计学意义(P=0.042),二甲双胍组患者维生素B12缺乏率为26.87%(36/134),显著高于对照组的17.94%(68/379),差异有统计学意义(P=0.033);按二甲双胍应用剂量分组,低剂量组、中剂量组和高剂量组患者血清维生素B12水平的差异无统计学意义(P=0.477);按二甲双胍用药时间分组,1年组、1~2年组和3年及以上组患者血清维生素B12水平的差异有统计学意义(P=0.035),1年组与3年及以上组患者血清维生素B12水平的差异有统计学意义(P=0.035)。结论:口服二甲双胍的2型糖尿病患者血清维生素B12水平明显低于未应用二甲双胍患者,发生维生素B 12缺乏比例明显高于未应用二甲双胍患者,血清维生素B 12水平受二甲双胍应用时间的影响。OBJECTIVE:To probe into the effect of oral metformin on serum vitamin B12 levels in patients with type 2 diabetes.METHODS:Medical records of patients with vitamin blood concentration monitoring from Aug.2015 to Dec.2018 were collected through the HIS system of China-Japan Friendship Hospital(hereinafter referred to as“our hospital”)for retrospective analysis,according to patient's medication,it was divided into metformin group and control group,the serum vitamin B12 concentrations of the two groups were compared;serum vitamin B12<200 pg/ml was set as the limit to compare the deficiency of vitamin B1 between two groups;the medical records were divided into low dose group,medium dose group and high dose group according to different dose of metformin,the medical records were divided into 1-year group,group of 1-2 years,group of 3 years and above according to the duration of metformin administration,and the differences of serum vitamin B12 concentration was compared respectively.RESULTS:A total of 134 cases were involved in metformin grou and 379 cases were involved in control group,the difference in serum vitamin B12 concentrations between two groups was statistically significant(P=0.042),the deficiency rate of vitamin B12 of metformin group was 26.87%(36/134),which was significantly higher than that of the control group(17.94%,68/379),with statistically significant difference(P=0.033);there was no statistical significance in differences of serum vitamin B12 concentration among the low dose group,middle dose group and high dose groups according to the dose of metformin(P=0.477);according to the duration of metformin administration,the differences of serum vitamin B12 concentration among 1-year group,group of 1-2 years and group of 3 years and above were statistically significant(P=0.035),the difference between 1-year group and group of 3 years and above were also statistically significant(P=0.035).CONCLUSIONS:The serum vitamin B12 concentration of type 2 diabetes patients with oral metformin was significantly lo
