无保护会阴助产初产妇会阴裂伤的危险因素分析

摘　　要：目的分析无保护会阴助产初产妇会阴裂伤的危险因素。方法选择2017年6月至2018年6月于本院经阴道分娩初产妇391例为研究对象,均行无保护会阴助产。统计初产妇会阴裂伤发生情况,采用单因素和多因素Logistic回归分析探讨无保护会阴助产初产妇会阴裂伤的危险因素。结果391例无保护会阴助产经阴道分娩的产妇中,102例发生会阴裂伤,发生率为26.09%。单因素分析结果显示,产前会阴是否水肿、助产士工作年限、新生儿体重、产力、胎头硬度均为无保护会阴助产初产妇会阴裂伤的影响因素(均P<0.05)。多因素Logistic回归分析显示,产前会阴水肿、助产士工作年限<5年、新生儿体重≥3.0 kg、产力过强均为无保护会阴助产初产妇会阴裂伤的危险因素(均P<0.05)。结论无保护会阴助产初产妇会阴裂伤受多因素影响,应针对危险因素尽早采取针对性预防措施,降低产妇会阴裂伤发生率,提高产妇生活质量。Objective To explore the risk factors of perineal laceration in unprotected perineal midwifery.Method 391 women who were vaginal delivery in our hospital from June 2017 to June 2018 were selected as the study subjects,all of whom underwent unprotected perineal midwifery.The incidence of perineal laceration in primipara was analyzed.The risk factors of perineal laceration in primipara were analyzed by single factor and multi factor Logistic regression analysis.Result Among 391 who were vaginal delivery by unprotected perineal midwifery,102 cases suffered from perineal laceration,the incidence was 26.09%.The results of univariate analysis showed that whether the perineum was edema before delivery,the working years of the midwife,the weight of the newborn,the labor force and the hardness of the fetal head were all the influencing factors of perineum laceration in unprotected primipara midwifery(all P<0.05).Multivariate Logistic regression analysis showed that perineum edema,working years of midwife<5 years,weight of newborn≥3.0 kg,and labor force were all the risk factors of perineum laceration in unprotected primipara midwifery(all P<0.05).Conclusion The perineum laceration of unprotected perineal midwifery is affected by many factors.We should take targeted preventive measures as soon as possible to reduce the incidence of perineum laceration and improve the quality of life of the primipara.
