关节镜手术治疗膝关节内侧半月板Ramp损伤的疗效研究

摘　　要：目的研究关节镜手术治疗膝关节内侧半月板Ramp损伤的疗效。方法选取2018年6月至2019年6月期间延安市人民医院骨关节运动损伤科收治的60例膝关节内侧半月板Ramp损伤患者作为研究对象,所有患者均接受关节镜手术治疗。比较治疗前后患者的膝关节活动度、视觉模拟评分法(VAS)、膝关节功能评分(Lysholm)、膝关节运动评分(Tegner)、膝关节功能主观评分(IKDC)、膝关节恢复及并发症发生情况。结果治疗后,患者的膝关节活动度为(133.92±9.11)°,明显高于治疗前的(119.18±9.23)°,而VAS评分为(2.04±0.33)分,明显低于治疗前的(5.27±0.59)分,治疗前后比较差异均具有统计学意义(P<0.05);治疗后,患者的Lysholm、Tegner及IKDC评分分别为(93.10±6.23)分、(6.58±1.01)分及(87.13±2.14)分,分别与治疗前的(40.31±2.19)分、(3.18±0.12)分和(45.06±1.94)分比较明显升高,差异均有统计学意义(P<0.05);治疗后,患者膝关节恢复优良率为91.7%,明显高于治疗前的66.7%,差异有统计学意义(P<0.05);治疗后有4例患者出现并发症,并发症发生率为6.7%。结论关节镜手术治疗膝关节内侧半月板Ramp损伤可明显减轻患者疼痛,改善膝关节功能,减少并发症,安全可靠,在临床中值得推广应用。Objective To study curative effect of arthroscopic surgery in patients with meniscal ramp lesions of the knee.Methods A total of 60 patients with meniscal ramp lesions of the knee,who admitted to Department of Bone and Joint Sports Injuries,Yan'an People's Hospital from June 2018 to June 2019,were selected and received arthroscopic surgery.Knee range of motion,visual analog scale(VAS)pain score,Lysholm score,Tegner score,International Knee Documentation Committee(IKDC)score,knee joint recovery,and complications of the patients were observed before and after treatment.Results After treatment,the knee range of motion of the patients was(133.92±9.11)°,which was significantly rather than(119.18±9.23)°before treatment,while VAS score was 2.04±0.33,which was significantly lower than 5.27±0.59 before treatment(all P<0.05);Lysholm score,Tegner score,and IKDC score of the patients were 93.10±6.23,6.58±1.01,87.13±2.14,respectively,which were significantly higher than corresponding(40.31±2.19),3.18±0.12,45.06±1.94 before treatment(all P<0.05);excellent rate of knee joint recovery of patients was 91.7%,which was significantly higher than 66.7%before treatment(P<0.05);After treatment,complications occurred in 4 patients,and the incidence of complications was 6.7%.Conclusion Arthroscopic surgery has significant effect on meniscal ramp lesions of the knee.It can obviously relieve pain,improve knee function,reduce complications,and is safe and reliable.It is worth popularizing and applying in clinic.
