罗哌卡因复合舒芬太尼腰麻在剖宫产术中的临床应用效果观察

摘　　要：目的:探究罗哌卡因复合舒芬太尼腰麻在剖宫产术中的应用效果。方法:选取2018年1月至2019年1月在我院进行剖宫产术产妇120例,按照随机数字法将其分为研究组和对照组,各60例,其中对照组产妇单纯给予罗哌卡因腰麻,研究组产妇给予罗哌卡因复合舒芬太尼腰麻,观察对比2组产妇的药物起效时间、痛觉恢复时间、运动恢复时间、麻醉效果、术中血流动力学指标、新生儿Apgar评分、麻醉后相关并发症以及术后24 h QoR-40量表评分。结果:研究组药物起效时间显著短于对照组(P<0.05),而痛觉恢复时间以及运动恢复时间明显长于对照组(P<0.05);研究组麻醉效果优良率明显高于对照组(P<0.05);研究组和对照组新生儿Apgar评分差异无统计学意义(P>0.05);研究组麻醉后5 min的血流动力学指标与对照组比较差异有统计学意义(P<0.05),其血流动力学更加平稳;对照组相关并发症发生率显著高于研究组(P<0.05);术后24 h QoR-40量表评分研究组恢复质量显著高于对照组(P<0.05)。结论:罗哌卡因复合舒芬太尼腰麻可以显著提高麻醉效果,稳定血流动力学,同时可以显著提高剖宫产术后恢复质量,值得在临床上进一步采用和推广。Objective: To explore the effect of ropivacaine combined with sufentanil in spinal anesthesia for cesarean section.Methods: A total of 120 cases of cesarean section from Jan 2018 to Jan 2019 in our hospital were selected and divided into study group and control group according to random number method, 60 cases in each group. The control group was given ropivacaine only,while the study group was given ropivacaine combined with sufentanil. The drug onset time, pain recovery time, motor recovery time,anesthesia effect,intraoperative hemodynamic parameters, neonatal Apgar score, post-anesthesia complications and QoR-40 score at24 after operation in the two groups were observed and compared. Results: The drug onset time in the study group was significantly shorter than that in the control group(P<0.05), while the pain recovery time and motor recovery time were significantly longer than those in the control group(P<0.05).The excellent and good rate of anesthesia effect in the study group was significantly higher than that in the control group(P<0.05).There was no significant difference in the neonatal Apgar score between the two groups(P>0.05).Compared with the control group, the intraoperative hemodynamic indexes were significantly different after 5 minutes of anesthesia(P<0.05), and the hemodynamics was more stable in the study group. The incidence of related complications in the control group was significantly higher than that in the study group(P<0.05). The QoR-40 scale score at 24 h after operation in the study group was significantly higher than that in the control group(P<0.05). Conclusion: Ropivacaine combined with sufentanil can significantly improve the anesthetic effect, stabilize the hemodynamics, and significantly improve the quality of the postpartum recovery, which is worthy of further clinical application and promotion.
