摘  要:目的探讨1例全麻术中羟乙基淀粉(HES)130/0.4氯化钠注射液导致过敏性休克病例的临床特点以及HES目前在国内外应用的新进展。方法1例老年男性患者于2019年1月16日在全身麻醉下行腹腔镜下胃病损切除术时,静脉滴注HES致过敏性休克,按照过敏性休克及时处理,患者循环恢复并稳定完成手术;通过查找相关文献学习HES的药物特点并了解HES临床应用的新进展。结果该患者经过积极的抢救,生命体征恢复平稳,手术顺利完成,术后无相关不良反应。结论HES在围手术期应用应权衡利弊,严格把控其适应症及禁忌症,熟练掌握过敏性休克的抢救流程对每个麻醉医生都至关重要。Objective To investigate the clinical features of a case of anaphylactic shock caused by hydroxyethyl starch(HES)130/0.4 sodium chloride injection in general anesthesia and the new progress in application of HES at home and abroad. Methods On January 16,2019,an elderly male patient underwent laparoscopic gastrectomy under general anesthesia. Anaphylactic shock occurred during intravenous drip of HES,Through timely treatment according to anaphylactic shock,the patient recovered stable circulation and passed the operation safely. The author studied the drug characteristics of HES and understood the new progress of clinical application of HES by searching relevant literatures. Results After active rescue,the patient′s vital signs recovered stably,the operation was successfully completed,and there was no related adverse drug reaction after the operation. Conclusion In the application of HES in perioperative period,the doctors should weigh the advantages and disadvantages,strictly control its indications and contraindications as well,mastering the rescue process of anaphylactic shock is crucial for every anesthesiologist.
