血栓弹力图在妊娠合并血小板减少症患者中的应用

摘　　要：目的探讨血栓弹力图在妊娠合并血小板减少症患者中的应用价值。方法随机选择2016年4月—2019年1月入住医院的妊娠合并血小板减少症患者50例为观察组,按照血小板水平分成甲乙两个亚组,甲组:血小板<50×10^9/L,乙组:血小板(50~99)×10^9/L,两组患者各25例;同时设置同一阶段接受诊治的50例健康孕妇为对照组,对入组者进行血栓弹力图检测,比较各组凝血功能和血栓弹力图检测结果。结果观察组和对照组患者的凝血酶时间TT、纤维蛋白原FIB之间不存在统计学差异(P>0.05),观察组血小板为(53.09±24.62)×10^9/L,明显低于对照组的(182.55±39.26)×10^9/L,活化部分凝血活酶时间APTT和凝血酶原时间PT明显高于对照组,存在统计学差异(P<0.05);甲组血小板为(29.47±15.63)×10^9/L,凝血时间R为(8.12±2.49)min,最大振幅MA为(55.05±7.86)mm,凝血综合指数CI为(1.08±0.85)mmol/L,与乙组相比,存在统计学差异(P<0.05)。结论血栓弹力图在妊娠合并血小板减少症患者的凝血功能监测中发挥着重要作用,可明显提高该疾病的诊断准确性,推广应用价值明显。Objective To explore the applicability of thromboelastography in pregnancy with thrombocytopenia.Methods Fifty pregnant women with thrombocytopenia admitted to our hospital from April 2016 to January 2019 were randomized as observation group and they were equally divided into two groups according to platelet level:group A with platelet<50×10^9/L,group B with platelet between(50~99)×10^9/L.Meanwhile,50 healthy pregnant women who received treatment in our hospital at the same stage were set as control group.Thromboelastography was used to detect participants and two groups were compared in terms of the coagulation function and the Results of thromboelastography.Results There was no significant difference in thrombin time(TT)and fibrinogen(FIB)between the control group and the observation group(P>0.05).The platelet in the observation group was(53.09±24.62)×10^9/L,which was significantly lower than that in the control group(182.55±39.26)×10^9/L.Activated partial thromboplastin time(APTT)and prothrombin time(PT)were significantly higher than those in the control group(P<0.05).In group A,the platelet count was(29.47±15.63)×10^9/L;the coagulation time R was(8.12±2.49)min;the maximum amplitude(MA)was(55.05±7.86)mm;and the coagulation index(CI)was(1.08±0.85)mmol/L.There was a statistical difference compared with group B(P<0.05).Conclusion Thromboelastography plays an important role in monitoring the coagulation function of pregnant patients with thrombocytopenia.It can obviously improve the diagnostic accuracy of the disease and has promising applicable value.
