摘 要:目的探讨胃十二指肠动脉的形态及变异。方法回顾性分析昆明医科大学第二附属医院肝胆胰外科二病区2015年1月至2017年12月行3D重建软件重建上中腹动脉系统74例并进行统计学分析。结果通过3D可视化观察,发现74例中最常见解剖为肝总动脉发出胃十二指肠动脉(GDA),共53例,GDA长度(23.8±9.63) mm,其中GDA长度<15 mm,14例;GDA长度>15 mm,39例,包含4种亚型:I型,胃右动脉发自肝固有动脉;Ⅱ型,胃右动脉发自肝左动脉;Ⅲ型,胃右动脉发自胃十二指肠动;Ⅳ型,胃右动脉未显示。其余21例归入变异形态,GDA长度(9.2±4.77) mm,其中所有GDA长度均小于15 mm。GDA长短与GDA发生变异具有相关性(P<0.01)。结论 3D可视化技术有利于胃十二指肠动脉的形态学观察,利于发现其解剖变异;GDA长度<15 mm时,出现变异的可能性更大。Objective To investigate the morphology and variation of gastroduodenal artery. Methods A retrospective analysis of 74 cases of the upper abdominal artery system was performed in the second ward of the Department of Hepatobiliary Surgery, the Second Affiliated Hospital of Kunming Medical University from January2015 to December 2017. The results were statistically analyzed. Re s ults Through 3 D visualization, the most common anatomy found in the 74 cases was the gastroduodenal artery(GDA) originating from the common hepatic artery, which consisted of 53 cases. The length of GDA was(23.8 ±9.63)mm. The GDA length was <15 mm in 14 cases and GDA length >15 mm in 39 cases. They were divided into 4 subtypes: in type I,the right gastric artery was from the proper hepatic artery;in type II, the right gastric artery was from the left hepatic artery;in type III, the right gastric artery was from the gastroduodenal movement;in type IV, the right gastric artery was not shown. The remaining 21 cases were classified as variants. The GDA length was(9.2 ± 4.77) mm,and all GDA lengths were less than 15 mm. The length of GDA was found to be related to the variation of GDA(P<0.01). Conclus ion 3 D visualization technology is beneficial to the morphological observation of the gastroduodenal artery, which is conducive to the discovery of its anatomical variation. When the GDA length is <15 mm,the possibility of variation is greater.
