摘　　要：目的探讨多学科团队合作模式在提高缺血性脑卒中患者出院准备度中的应用效果。方法选择2018年2月至2018年7月江油市人民医院收治的69例缺血性脑卒中患者为对照组,2018年8月至2018年12月收治的69例缺血性脑卒中患者为研究组,对照组在所患疾病临床治疗的基础上予以常规护理,研究组在对照组的基础上予以多学科团队合作综合护理,评价2组患者的出院准备度、患者自我管理行为以及对护理的满意度,并收集出院3个月内再入院情况。结果研究组出院准备度、患者自我管理行为得分、患者满意度均显著高于对照组(P<0.01);研究组较对照组3个月内再入院率低,差异具有统计学意义(P<0.05)。结论多学科团队合作模式能有效提高患者的出院准备度,改善患者自我管理行为,降低再入院率,提高患者满意度。Objective To explore the effect of multidisciplinary team cooperation on discharge readiness of patients with ischemic stroke.Me thods We selected 139 patients with ischemic stroke from our hospital from February to December,2018 and divided them into the experimental group and the control group according to the different nursing measures.The control group was treated with routine nursing care,while the experimental group with multidisciplinary team cooperation(MDT)mode intervention.Then the readiness for hospital discharge,self-management behavior,and patients’satisfaction were compared.Readmission data was collected within 3 months of discharge.Re s ults The readiness for hospital discharge,self-management behavior and patients’satisfaction scores were significantly higher in the experimental group than those in the control group(P<0.01).The readmission rate in the experimental group was significantly low in the control group(P<0.05).Conclus ion MDT intervention can effectively improve the readiness for hospital discharge,self-management behavior,reduce readmission rate and improve patients’satisfaction in the patients with ischemic stroke.
