孟鲁司特钠联合注射用细辛脑对慢性阻塞性肺疾病急性加重期患者的效果及对血清炎性指标水平的影响

摘　　要：目的探究孟鲁司特钠联合注射用细辛脑对慢性阻塞性肺疾病急性加重期(AECOPD)患者的效果及对血清炎性指标水平的影响。方法选择我院2017年1月至2019年1月收治的78例AECOPD患者作为研究对象,采用抽签法随机分为观察组(39例,基础治疗+孟鲁司特钠+注射用细辛脑)和对照组(39例,基础治疗+孟鲁司特钠)。比较两组患者的临床疗效、血清炎性指标水平、肺功能及不良反应发生情况。结果观察组的治疗总有效率明显高于对照组(P<0.05)。治疗后,两组的血清CRP、IL-12、PCT水平均明显降低,IL-10水平明显升高,且观察组各项血清炎性指标水平均明显优于对照组(P<0.05)。治疗后,两组的FVC、FEV1、FEV1/FVC以及PaO2/FiO2升高,PaCO2降低,且观察组上述指标均明显优于对照组,差异具有统计学意义(P<0.05)。两组的恶心、腹部不适、呕吐发生率比较,差异无统计学意义(P>0.05)。结论孟鲁司特钠联合注射用细辛脑对AECOPD患者的效果显著,可改善患者的肺功能,可能与其调节血清炎性指标水平,从而抑制患者炎症反应有关。Objective To investigate the effect of montelukast sodium combined with asarone for injection on patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and its influence on serum inflammatory indexes levels. Methods A total of 78 patients with AECOPD admitted in our hospital from January 2017 to January 2019 were selected as research objects and randomly divided into observation group(39 cases, basic treatment +montelukast sodium +asarone for injection) and control group(39 cases, basic treatment +montelukast sodium) by drawing lots. The clinical efficacy, serum inflammatory indexes levels, pulmonary function and adverse reactions were compared between the two groups. Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05). After treatment, the levels of serum CRP, IL-12, PCT in the two groups significantly reduced, the level of IL-10 increased significantly, and the levels of serum inflammatory indexes in the observation group were significantly better than those in the control group(P<0.05). After treatment, the FVC, FEV1, FEV1/FVC, PaO2/FiO2 in the two groups significantly increased, the PaCO2 decreased, and those in the observation group were significantly better than the control group(P<0.05). There were no significant differences in the incidences of nausea, abdominal discomfort and vomiting between the two groups(P>0.05). Conclusion Montelukast sodium combined with asarone for injection on patients with AECOPD has a significant effect, can improve patients’ lung function. It may be related to regulating the levels of serum inflammatory indexes, so as to inhibit the inflammatory response of patients.
