摘　　要：目的分析不同二氧化碳(CO2)气腹压力对腹腔镜下胆总管切开取石术患者应激反应的影响。方法回顾性分析我院行腹腔镜下胆总管切开取石术的96例患者的临床资料,根据CO2气腹压力的不同将患者分为A组(n=31,14~15 mmHg)、B组(n=33,>13 mmHg且<14 mmHg)、C组(n=32,12~13 mmHg)。比较三组患者不同时刻的应激指标水平及住院时间。结果 T1、T2时刻,C组的Cor、NE、CRP、PGE2水平均低于A组及B组,且B组低于A组(P<0.05);T1时刻,三组的Cor、NE、CRP、PGE2水平均高于T0、T2时刻(P<0.05);T2时刻,三组的Cor、NE、CRP、PGE2水平均高于T0时刻(P<0.05)。C组住院时间短于A组及B组,且B组短于A组(P<0.05)。结论低CO2气腹压力应用于腹腔镜下胆总管切开取石术中可减轻患者应激反应,缩短住院时间。Objective To analyze the effect of different CO2 pneumoperitoneum pressure on stress response of patients undergoing laparoscopic choledocholithotomy. Methods The clinical data of 96 patients who underwent laparoscopic choledocholithotomy in our hospital were analyzed retrospectively. According to the different pressure of CO2 pneumoperitoneum, the patients were divided into group A(n=31, 14-15 mmHg), group B(n=33, >13 mmHg and <14 mmHg),and group C(n=32, 12-13 mmHg). The levels of stress indexes in different points and hospitalization time of the three groups were compared. Results At T1 and T2, the levels of Cor, NE, CRP and PGE2 in the group C were lower than those in the group A and B, and those in the group B were lower than the group A(P<0.05);At T1, the levels of Cor, NE, CRP and PGE2 in the three groups were higher than those at T0 and T2(P<0.05);at T2, the levels of Cor, NE, CRP and PGE2 in the three groups were higher than those at T0(P<0.05). The hospitalization time in the group C was shorter than that in the groups A and B,and that in the group B was shorter than the group A(P<0.05). Conclusion The application of low CO2 pneumoperitoneum pressure in laparoscopic choledocholithotomy can reduce the stress response of patients and shorten the hospital stay.
