摘 要:目的探究高脂、高蛋白饮食对肾缺血再灌注损伤后肾功能的影响。方法构建大鼠肾缺血再灌注模型(模型组),分别给予标准饲料、高脂饲料、高蛋白饲料和高脂高蛋白饲料连续饲养2 w,检测期间大鼠血肌酐(Cr)、尿素氮(BUN)和尿酸(UA)水平及变化趋势。2 w后取肾脏组织切片,观察比较各组肾组织形态学的变化。结果在术后第1、3、7和14 d,对照组各亚组的血Cr、BUN和UA水平基本均无显著变化(P> 0.05)。而在模型组中,标准组血Cr、BUN和UA水平虽高于对照组,但各时点间比较无显著差异(P> 0.05);但其他3组随着饲养时间的延长,血Cr、BUN和UA水平均逐渐升高,显著高于同时点标准组和对照组(P <0.01),且均是以高脂高蛋白组最高,高脂组与高蛋白组基本相同,标准组最低(P <0.01)。模型组的肾小球结构异常,出现肾损伤病理学表现,也是以高脂高蛋白组最显著,高脂组与高蛋白组次之,标准组最少。结论肾缺血再灌注损伤后,高脂、高蛋白饮食会对肾脏造成进一步的损伤,尤其是高脂高蛋白饮食的危害更大。Objective To explore the impact of high-fat and high-protein diet on renal function in rats with ischemia-reperfusion injury. Methods The rat ischemia-reperfusion models(model group) were constructed. The rats were fed with standard diet, high-fat diet, high-protein diet and high-fat and high-protein diet respectively for two weeks. The levels and changes of serum creatinine(Cr),blood urea nitrogen(BUN) and uric acid(UA) were detected during this period. Two weeks later, the renal tissue sections were taken and used to observe the morphological changes of renal tissues. Results On Day one, three, seven and 14 post treatment, there was no change in Cr, BUN or UA levels in all control subgroups(P > 0.05). In the model group, the levels of Cr, BUN and UA in the standard subgroup were higher than the control group, with no significant difference at each time point(P > 0.05);the levels of Cr,BUN and UA in the rest three subgroups increased with the feed time, significantly higher than the control and standard subgroups at each time point(P < 0.01). The high-fat and high-protein subgroup topped in above three levels, followed by the high-fat subgroup(same as high-protein subgroup) and standard subgroup(P < 0.01). Abnormality was found in the glomerulus structure in the model groups, with a pathological manifestation of renal injury, which was more significant in the high-fat and high-protein subgroup,followed by the high-fat subgroup(same as high-protein subgroup) and standard subgroup. Conclusion The high-fat and high-protein diet will aggravate the renal injury in rats with ischemia-reperfusion injury, especially when used together.
