三种长度PFNA固定股骨转子间骨折的手术创伤评估的研究

摘　　要：目的:比较3种不同长度PFNA髓内固定住院期间的手术指标。方法:2014年8月至2015年8月期间骨科收治的老年股骨粗隆间骨折且行PFNA髓内固定患者,分别采用3种不同长度PFNA固定(短主钉组:170 mm,中主钉组:200 mm,长主钉组:240 mm),对患者的手术切口长度、手术时间、术中出血、住院天数进行评估。结果:所有患者的手术切口长度、手术时间、术中出血量、住院天数方面比较,中主钉、长主钉组患者均高于短主钉组(P<0.05)。结论:主钉长度较长的PFNA确实能减少应力的集中,降低术后继发骨折的发生率,然而较长的主钉也会引起较大的手术创伤,长主钉的PFNA对于临床适应症的选择仍需严格控制。Objective:To compare the clinical effects among 3 kinds of proximal femoral nail antirotation(PFNA)with different lengths among patients with intertrochanteric fractures during hospitalization.Methods:3 kinds of PFNA with different lengths were fixed to 3 groups of people hospitalized from Aug.2014 to Aug.2015(Group A:170 mm,Group B:200 mm,Group C:240 mm),with the length of surgical incision,the operation time,hemorrhage during operation and the lengths of hospital stay evaluated.Results:The lengths of surgical incision,the operation time,and the lengths of hospital stay in Group B and Group C were significantly longer than those in Group A,and the hemorrhage during operation in Group B and Group C was significantly less than that in Group A.Conclusion:Longer PFNA does ease stress concentration and reduce the rates of secondary fractures.However,longer PFNA causes larger surgical trauma.Therefore,the surgical indications of longer PFNA still need strict control.
