腹膜透析患者近期发生心脑血管病事件的危险因素分析

摘　　要：目的分析腹膜透析患者近期内发生心脑血管病事件的危险因素。方法回顾分析2010年6月至2019年6月肾内科腹膜透析伴发生心脑血管病事件患者的临床资料。根据腹膜透析≤1年内是否发生心脑血管病事件,将患者分为1年内发生心脑血管病事件组和1年内未发生心脑血管病事件组,采用Cox比例回归模型分析影响腹膜透析患者近期发生心脑血管病事件的危险因素。结果 352例腹膜透析患者中1年内发生心脑血管病事件133例,未发生心脑血管病事件219例。多因素Cox回归分析表明:糖尿病、年龄、血白蛋白、红细胞分布宽度、高密度脂蛋白胆固醇、血磷、叶酸、同型半胱氨酸与腹膜透析患者近期发生心脑血管病事件密切相关(HR分别为3.872、1.158、0.737、1.226、1.362、0.829、0.715和1.096,P<0.05或0.01)。结论糖尿病是腹膜透析患者近期发生心脑血管病事件的主要危险因素。Objective To analyze the risk factors of recent cardiovascular and cerebrovascular events(CCEs)in patients underwent peritoneal dialysis. Methods clinical data of peritoneal dialysis patients with CCEs in nephrology department from June 2010 to June2019 were retrospectively analyzed. Patients were divided into group with(CCE group)and without(non-CCE group)CCEs occurred within 1 year after peritoneal dialysis. Cox proportional regression model was used to analyze the risk factors affecting the recent CCEs in peritoneal dialysis patients. Results Of 352 peritoneal dialysis patients, 133 cases had CCEs and 219 cases had no CCEs within one year after peritoneal dialysis. Multivariate Cox regression analysis showed that diabetes mellitus, age, blood albumin, erythrocyte distribution width, high-density lipoprotein cholesterol, blood phosphorus, folic acid and homocysteine were closely related to the CCEs of patients with peritoneal dialysis(HR=3.872, 1.158, 0.737, 1.226, 1.362, 0.829, 0.715, 1.096, respectively, P <0.05 or 0.01). Conclusion Diabetes mellitus is the main risk of recent CCEs in patients underwent peritoneal dialysis.
