摘 要:目的探讨3D腹腔镜胆总管探查、取石一期缝合治疗肝外胆管结石的效果。方法将2018-01—2019-01间在郑州大学第一附属医院接受腹腔镜胆总管探查、取石一期缝合术的68例肝外胆管结石患者分为3D组(33例)和2D组(35例)。比较2组患者的手术时间,以及术中出血量、胆囊切除时间、胆囊破损率、胆总管缝合时间和术后并发症发生率。结果2组的术中出血量、胆囊破损率,以及术后并发症发生率、住院时间和住院费用差异均无统计学意义(P>0.05)。3D组的手术时间、胆囊切除时间,以及胆总管缝合时间均优于2D组,差异有统计学意义(P<0.05)。结论与2D腹腔镜比较,3D腹腔镜胆总管探查、取石一期缝合治疗肝外胆管结石,能有效缩短手术时间,加快胆囊切除和胆总管切口缝合速度,优势明显。Objective To investigate the effect of 3D laparoscopic common bile duct exploration and stone extraction with primary suture in the treatment of extrahepatic choledocholithiasis.Methods Collected data of extra hepatic bile duct stones patients from January 2018 to January 2019,laparoscopic cholecystectomy and choledochotomy combined with primary closure were performed in all patients.According to their surgical methods,they were divided into 3D laparoscopic group(n=33)and 2D laparoscopic group(n=35).The differences in hospital time,operation time,intraoperative blood loss,gallbladder resection time,gallbladder damage rate,bile duct suture time and postoperative complications were compared between the two groups.Results The operation time,cholecystectomy time and common bile duct suture time of the 3D group were all better than that of the 2D group,with statistically significant differences(P<0.05).There were no statistically significant differences in postoperative hospitalization duration,hospitalization cost,intraoperative blood loss,gallbladder damage cases and complications between the two groups(P>0.05).Conclusion Compared with 2D laparoscopy,3D laparoscopy has obvious advantages in the one-stage suture of extrahepatic bile duct stones,which can effectively shorten the operation time and accelerate the incision suture speed,and is worth promoting in clinical surgery.
