瘢痕断层皮片+脱细胞异体真皮治疗大面积深Ⅱ度烧伤瘢痕的价值分析

摘　　要：目的:分析瘢痕断层皮片联合脱细胞异体真皮治疗大面积深Ⅱ度烧伤瘢痕的临床价值.方法:分析研究对象为医院收治的25例大面积深Ⅱ度烧伤瘢痕患者,以自身对照方式进行分组研究,切除瘢痕后,右侧关节厚0.7mm瘢痕断层皮片为对照组(n=30)进行修复,左侧关节厚0.5mm的瘢痕断层皮片+脱细胞异体真皮为观察组(n=30)进行修复,比较2组瘢痕修复效果.结果:观察组的Ⅰ期、Ⅱ期愈合率、术后感染、不同时间愈合例数以及手术时间、创面愈合时间与对照组相比无差异,无统计学意义(P>0.05),观察组的创面愈合VSS评分、关节功能恢复优良率与对照组相比差异显著(P<0.05).结论:瘢痕断层皮片联合脱细胞异体真皮治疗大面积深Ⅱ度烧伤瘢痕安全可行,值得广泛应用.Objective:To analyze the clinical value of scar sectional skin graft combined with acellular allogenic dermis in the treatment of large area deep second degree bum scar.Methods:25 patients with large area deep second degree bum scar were divided into two groups by self-control method.After scar removal,the right joint thickness of 0.7mm scar sectional skin was repaired as the control group(n=30),while the left joint thickness of 0.5mm scar sectional skin and acellular allogenic dermis were repaired as the observation group(n=30).The scar repair was compared between the two groups.Results:There was no significant difference in the healing rate of stage I and II,infection after operation,healing cases at different time,operation time and wound healing time between the observation group and the control group(P>0.05).There was significant difference in the VSS score of wound healing and the excellent and good rate of joint function recovery between the observation group and the control group(P<0.05).Conclusion:Scar sectional skin graft combined with acellular allogenic dermis was safe and feasible for the treatment of large area deep second degree bum scar,and it was worth wide application.
