第1掌骨桡背侧带蒂皮瓣与筋膜蒂皮瓣修复示指末节指腹皮肤软组织缺损的疗效比较

摘　　要：目的:分析对比用第1掌骨桡背侧带蒂皮瓣与筋膜蒂皮瓣修复示指末节指腹皮肤软组织缺损的临床疗效.方法:2013年3月-2018年6月,选取示指末节指腹皮肤软组织缺损的52例患者,随机分为2组,每组26例.观察组采用第1掌骨桡背侧带蒂皮瓣修复,对照组采用筋膜蒂皮瓣修复,观察2组患者的临床疗效.结果:术后48例皮瓣成活良好,观察组皮瓣全部成活,对照组皮瓣坏死4例,成活的皮瓣伤口愈合佳,全部患者术后4-12个月随访,观察组皮瓣术后患指耐寒程度、整体外观、耐磨性及感觉恢复情况都较对照组皮瓣好.治疗后2组疗效对比,差异具有统计学意义(P<0.05).结论:用第1掌骨桡背侧带蒂皮瓣修复示指末节指腹皮肤软组织缺损综合疗效优于筋膜蒂皮瓣.Objective:To compare and analyze the clinical effect of the first radial dorsal metacarpal pedicle skin flap with the fascia pedicle flap in the repair of the soft tissue defect of the abdominal skin in the end of index finger.Methods:From March 2013 to June 2018,Selection of 52 patients with soft tissue defect of abdominal skin at the end of index finger,randomly divided into A group and B group,each group with 26 cases.The A group was operated by the first radial dorsal metacarpal pedicle skin flap,and the B group was operated by the fascia pedicle flap.Observed and compared the clinical effects of two groups.Results:48 cases of postoperative flaps survived well,Group A flap all survives,4 cases of skin flap necrosis in Group B,Survival of the flap wound healing,All patients with postoperative 4-12 months follow-up.The degree of cold resistance,overall appearance,wear resistance and sensory recovery of Group A flap were better than that of Group B flap.Compared with the curative effect of the two groups after treatment,the difference was statistically significant,P<0.05.Conclusion:With the first radial dorsal metacarpal pedicle skin flap to repair of the soft tissue defect of the abdominal skin in the end of index finger in the comprehensive curative effect was better than the fascia pedicle flap.
