血清降钙素原在早期诊断系统性红斑狼疮合并肺部感染中的应用

摘　　要：目的:探讨降钙素原在早期鉴别系统性红斑狼疮合并肺部感染与系统性红斑狼疮合并狼疮性肺炎的作用。方法:采用回顾性病例对照分析的方法,将中山大学孙逸仙纪念医院风湿科2017年1月-2017年12月住院的有发热的系统性红斑狼疮合并肺部感染患者纳入实验组(n=9),发热的系统性红斑狼疮合并狼疮性肺炎的患者纳入对照组(n=11),比较两组间降钙素原、入院24小时内最高体温、血白细胞计数与中性粒细胞比例、C反应蛋白和抗核抗体的水平。结果:实验组的血清降钙素原水平明显高于对照组[(1.730±2.66)ng/L vs(0.095±0.083)ng/L,P<0.05],两组间入院24小时内最高体温、血白细胞计数与中性粒细胞比例、C反应蛋白和抗核抗体的水平均无显著性差异(P>0.05)。结论:血清降钙素原有利于早期鉴别系统性红斑狼疮合并肺部感染与系统性红斑狼疮合并狼疮性肺炎。Objective:To explore application of serum procalcitonin in early diagnose of patients with systemic lupus erythematosus complicated with pneumonia. Methods:The clinical data of patients with systemic lupus erythematosus complained of fever were analyzed retrospectively from Jan 2017 to Dec 2017 in department of rheumatism,Sun Yat-sen memorial hospital of Sun Yat-sen university. The clinical data of 9 patients with systemic lupus erythematosus complicated with pneumonia(experiment group,n=9),while data of 11 patients with systemic lupus erythematosus complicated with lupus pneumonia(control group,n=11) were reviewed retrospectively. The serum procalcitonin,highest body temperature in 24 hours after admission,total white blood cell counting,level of serum C reactive protein and serum antinuclear antibody were compared between the two groups. Results:The serum procalcitonin level was significantly higher in case group than that in control group[(1.730±2.66)ng/L vs(0.095±0.083)ng/L,P<0.05]. The highest body temperature in 24 hours after admission,total white blood cell counting,serum C reactive protein lever and serum antinuclear antibody had no significant differences between the two groups. Conclusion:Serum procalcitonin may be useful in early diagnosis of systemic lupus erythematosus complicated with pneumonia.
