小剂量溶栓剂治疗糖尿病肾病的临床疗效评价

摘　　要：目的:探讨小剂量溶栓剂治疗糖尿病肾病的临床疗效。方法:以2017年6月-2018年6月在南方医科大学珠江医院诊治的糖尿病肾病患者80例为研究对象,并随机分为东菱精纯克栓酶组和尿激酶组,每组40例,分别给予东菱精纯克栓酶和尿激酶治疗,各组再根据白蛋白尿排泄量分为微量白蛋白尿和大量白蛋白尿组。比较两种溶栓药物在微量白蛋白尿和大量白蛋白尿组中的治疗效果及药物副作用情况。结果:东菱精纯克栓酶组中,大量白蛋白尿亚组及微量白蛋白尿亚组的白蛋白尿排放量均较治疗前明显下降(P<0.05)。尿激酶组中,微量白蛋白尿亚组的白蛋白尿排泄量虽有下降,但无明显差异(P>0.05),而大量白蛋白尿亚组的白蛋白尿排泄量明显下降(P<0.05)。结论:东菱精纯克栓酶的抗糖尿病肾病作用比尿激酶显著,可有效减少糖尿病肾病患者的白蛋白尿排泄量,且小剂量溶栓药物对微量白蛋白尿的治疗效果更明显,对大量白蛋白尿的疗效较差。Objective:To investigate the clinical effect of low-dose thrombolytic agent on diabetic nephropathy.Methods:80 patients with diabetic nephropathy were selected and treated in our hospital from June 2017 to June 2018.They were randomly divided into Batroxobin group and Urokinase group treated by Batroxobin and Urokinase respectively,and each group was 40 cases. Each group was further divided into microalbuminuria and massive albuminuria according to urinary albumin excretion. The therapeutic effects and side effects of the two thrombolytic drugs in microalbuminuria and massive albuminuria were compared between the two groups. Results:Compared with before treatment by Batroxobin,the amount of albuminuria in large amount of urine albumin subgroup and microalbuminuria subgroup were significantly decreased,both P<0.05. Compared with before treatment by Urokinase,although the urinary albumin excretion of microalbuminuria subgroup decreased,but the difference was not significant,P>0.05,while the macroalbuminuria subgroup were significantly decreased,P<0.05. Conclusion:It can be seen that Batroxobin has a more significant anti-diabetic nephropathy effect than urinary hormone,which can effectively reduce the urinary albumin excretion of patients,and the therapeutic effect of small dose thrombolytic drugs on microalbuminuria is more obvious,while the therapeutic effect on massive albuminuria is relatively poor.
