摘  要:心肌缺血、心衰、心绞痛、心肌离子通道病变等状态下,遇到情绪激动、应激状态或其它交感张力异常增高时,或使用外源性肾上腺素时,体内儿茶酚胺类物质明显增多,会诱发快速心律失常,甚至出现室颤或猝死的情况。这与激动α与β受体、调节心肌离子通道功能、影响心肌动作电位,导致心肌兴奋性改变有密切关系。该文从心肌电生理的角度,分析了肾上腺素受体对心肌复极过程中主要离子通道功能的调节,以及对动作电位时程的影响,加深对儿茶酚胺类物质引发室性心律失常机制的理解。In some states such as myocardial ischemia,heart failure,angina pectoral and myocardial ion channel disease and so on,one person may catch with a rapid cardiac arrhythmia,even a generate ventricular fibrillation or a sudden death,especially in the case of emotional agitation,stress or sympathetic tone increasing abnormally,and exogenous administration of epinephrine,followed by a significant rising of catecholamine.All the above have a relationship with the sympathetic excitatory forαandβreceptors,the regulation of myocardial ion channel function,changing the myocardial action potential,and the myocardial excitability.From the perspective of myocardial electrophysiology,this paper analyzes the effect of adrenal receptors on the regulation of ion channels,which plays a main action in the myocardial depolarization process,as well as the effect on the action potential duration,aiming to deepen the understanding of the mechanism of catecholamine inducing ventricular arrhythmias.
