碱性成纤维细胞生长因子对犬穿透性角膜移植愈合效果的临床观察

摘　　要：为探索碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)滴眼对犬穿透性角膜移植术后角膜愈合的影响,选取36只健康犬接受角膜移植手术,并随机分为4组(n=9):手术对照组(A)、地塞米松滴眼组(B)、地塞米松+环孢菌素A滴眼组(C)、地塞米松+环孢菌素A+bFGF滴眼组(D)。所有组术后均使用妥布霉素点眼。在术前(0 d)和术后第、7、14、21、35天进行泪液和眼压测定;在术后第、7、14、21、35天借助裂隙灯对角膜浑浊度、水肿程度和新生血管情况进行评分,采用荧光素钠染色观察角膜缺损情况。各组犬泪液量在术后3~21 d增加(P<0.05),第35天时C、D两组下降至术前水平(P>0.05);各组犬眼压在术后3~21 d下降(P<0.05),第35天时B、C和D组恢复至术前水平(P>0.05);C、D两组的角膜混浊度、新生血管评分和角膜荧光素钠染色评分均低于A、B两组(P<0.05);第35天时D组的角膜混浊度和水肿评分低于C组(P<0.05)。以上结果表明,bFGF与地塞米松、环孢菌素A、妥布霉素联用可有效缓解角膜移植术后的炎症反应,抑制新生血管生成,改善角膜水肿和浑浊度,为临床上犬角膜移植的术后治疗提供参考。To explore the clinical effect of basic fibroblast growth factor(bFGF)on corneal healing in dogs after penetrating keratoplasty(PKP),thirty-six healthy dogs were selected as recipient dogs and were assigned to one of the four groups(n=9):the surgical control group(A),the dexamethasone group(B),the dexamethasone plus cyclosporine group(C),and the cotreatment group of dexamethasone,cyclosporine and bFGF(D).All the groups received the tobramycin eye drop postoperatively.The tear volume and the intra-orbital pressure(IOP)were measured before(0 d)and at 3,7,14,21,and 35 d after surgery.The assessments of corneal turbidity,edema,and neovascularization by silt lamp,and the assessment of corneal defect by fluorescein sodium staining was conducted at 3,7,14,21,and 35 d.As a result,the tear volumes increased(P<0.05)from 3 to 21 d in all the groups,and approximated(P>0.05)to preoperative level at 35 d in the groups C and D.The IOP decreased(P<0.05)from 3 to 21 d in all the groups,and approximated(P>0.05)to the preoperative level at 35 d in the groups B,C and D.The scores of corneal turbidity,edema,neovascularization and fluorescein sodium staining were higher(P<0.05)in the groups A and B than those in the groups C and D at most observed time points.The score of corneal edema was lower(P<0.05)in the group D at 35 d than that in the group C.These results indicated that bFGF,in combination with dexamethasone,cyclosporine,and tobramycin,could relieve the inflammation,neovascularization,the turbidity and edema of cornea in dogs after PKP.These observations may provide reference for the postoperative treatment of canine PKP.
