摘  要:目的:研究平喘汤对慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)的作用机制。方法:采用香烟烟熏联合气管内注射脂多糖方法建立COPD大鼠模型,造模成功后,将100只大鼠随机分为平喘汤低、中、高剂量组、模型组、阳性对照组,每组20只,空白对照组20只。空白对照组和模型组以蒸馏水灌胃,阳性对照组以地塞米松混悬液灌胃;其余治疗组以平喘汤灌胃。每组均灌胃30天,每日1次。30天后,观察大鼠一般情况;取血清,酶联免疫法检测血清中肿瘤坏死因子-α(Tumor Necrosis Factor,TNF-α)、血管内皮细胞生长因子(Vascular Endothelial Growth Factor,VEGF)、白细胞介素(Interleukin,IL)-8、IL-1β含量,Western-Blot检测大鼠肺组织血小板源性生长因子-B(Platelet-Derived Growth Factor-B,PDGF-B)的表达;同时进行苏木精-伊红染色(Hematoxylin-eosin staining,简称HE染色),病理组织观察;检测支气管肺泡灌洗液(Broncho Alveolar Lavage Fluid,BALF)中白细胞计数和分类。结果:与模型组比较,平喘汤各剂量组和阳性对照组大鼠血清中TNF-α、IL-8、IL-1β降低(P <0.01或P <0.05),且以高剂量组降低最为明显,VEGF含量升高(P <0.01或P <0.05);与阳性对照组比较,高剂量组大鼠血清中TNF-α含量明显减少(P <0.05),低剂量组、中剂量组与阳性对照组比较,差异无统计学意义。与模型组比较,阳性组、高剂量组肺泡灌洗液中白细胞总数明显降低(P <0.01或P <0.05),中剂量组和低剂量组无统计学意义,阳性组及高、中、低剂量组巨噬细胞比例均升高(P <0.05),嗜中性粒细胞百分比降低(P <0.05),肺组织中PDGF-B含量降低(P <0.01或P <0.05)。结论:平喘汤能够降低COPD模型大鼠血清中TNF-α、IL-8、IL-1β水平,增高VEGF水平,降低肺组织中PDGF-B含量,高剂量组还可以降低BALF中白细胞及中性粒细胞总数。证实平喘汤治疗COPD的作用机制可能是减少炎症介质生成和释放,抑制气道重塑,�Objective: To study the mechanism for treating chronic obstructive pulmonary disease(COPD) with the Pingchuan decoction(平喘汤). Methods: COPD rats model were established by injecting the cigarette smoke into the trachea and injecting the lipopolysaccharide into airway. 100 cases were divided into the Pingchuan decoction groups(including the low dose group, middle dose group and high dose group), model group and positive control group, 20 rats in each;and there were 20 rats in the blank control group. The blank control group and the model group were given distilled water by gavage;the positive control group was given was intragastrically administered with dexamethasone suspension;the other treatment groups were treated with the Pingchuan decoction. After 30 days, the general situation of rats was observed. The TNF-α, VEGF, IL-8, IL-1β was detected by enzyme linked immunoassay. The expression of PDGF-B in the Lung Tissue was detected by Western-Blot. Pathology changes were observed by HE stains. White blood cell counts and classification in BALF were measured. Results: Compared with the Model group, the contents of TNF-α, IL-8, IL-1β in the Pingchuan decoction groups and positive control group were decreased(P<0.01, P<0.05), especially in dose group model groups, and VEGE were increased(P<0.01 or P<0.05). Compared with the positive control group, TNF-α in the Pingchuan decoction high dose group was decreased(P<0.05);there was no statistically significant difference in TNF-α between the Pingchuan decoction middle dose group, low dose group and Positive control group. Compared with the model group, the contents of white blood cell count in positive control group and he Pingchuan decoction high dose group were decreased(P<0.01 or P<0.05);there was no statistically significant difference in TNF-α between the Pingchuan decoction middle dose group, low dose group and model group.
