自拟中药汤剂治疗高血压脑出血并发肺部感染的效果及对血清炎症因子的影响

摘　　要：目的:评估自拟中药汤剂治疗高血压脑出血合并肺部感染的临床疗效。方法:选取2015年7月-2018年8月于我院治疗的高血压脑出血并发肺部感染的患者46例,按照随机数表法分为西医组22例,中西医组24例。西医组给予常规治疗;中西医组采用常规治疗联合中药汤剂,疗程均为2周,评估各组疗效,血清炎症因子水平,动脉氧分压(Partial Pressure of Oxygen,PaO2)及二氧化碳分压(Partial Pressure of Carbon Dioxide,PaCO2),肺功能改变及中医证候积分。结果:中西医组总有效率(87.50%)显著高于西医组(68.18%),P <0.05,治疗后两组患者白介素(Interleukin,IL)-6、肿瘤坏死因子-α(Tumor Necrosis Factor,TNF-α)、超敏C反应蛋白(High Sensitivity C-reactive Protein,hs-CRP)水平较治疗前均降低(P <0.05),中西医组降低尤为显著(P <0.05)。中西医组患者同比西医组,治疗后PaO2显著上升,PaCO2显著下降(P <0.05)。同时用力呼气容积(Forced Expiratory Volume in 1 second,FEV1),用力肺活量(Forced Vital Capacity,FVC),FEV1/FVC及FEV1%较治疗前显著提升(P <0.05),中医证候积分显著降低(P <0.05)。结论:自拟中药汤剂治疗高血压脑出血并发肺部感染,疗效显著,安全可靠,可临床推广。Objective: To evaluate the clinical efficacy of self-made TCM decoction on serum inflammatory factor levels in patients with hypertensive intracerebral hemorrhage with pulmonary infection. Methods: According to the random number table method, 46 patients were divided into the western medicine group(22 cases) and the integrative medicine groups(24 cases). The western medicine group received conventional treatment;the integrative medicine group was treated with TCM decoction more. Results: The total efficiency in the integrative medicine group(87.50%) was significantly higher than 68.18% in the western medicine group(P<0.05). After treatment, IL-6, TNF-α and hs-CRP in groups were lower than that before treatment(P<0.05), especially those in the integrative medicine group(P<0.05). In the integrative medicine group, PaO2 was more increased, and PaCO2 was more decreased;FEV1, FVC, FEV1/FVC and FEV1% were significantly higher than before treatment(P<0.05);TCM syndrome scores were significantly lower(P<0.05). Conclusion: TCM decoction on hypertensive cerebral hemorrhage with pulmonary infection shows an obvious curative effect, is safe and reliable, and can be clinically promoted.
