喉罩全麻和气管插管全麻在成人日间腔镜疝修补术中的应用比较

摘　　要：目的比较在成人日间腔镜疝修补术中,分别使用喉罩全麻、气管插管全麻的麻醉效果。方法将梅州市人民医院2017年1月—2018年12月收治的60例接受日间腔镜疝修补术的患者随机分成观察组(n=30)与对照组(n=30),对照组予以气管插管全身麻醉,观察组予以喉管全身麻醉,观察两组麻醉效果。结果观察组在围术期各时间段血流动力学无显著变化,而对照组在气管插入即刻(T2)、拔管后即刻(T3)的收缩压、舒张压及心率上均较观察组同一时刻高(P<0.05);术后拔管时间、麻醉时间及丙泊酚用量上观察组均短于对照组(P<0.05);术后观察组并发症总发生率为3.33%,显著低于对照组总发生率的20.00%(P<0.05)。结论在成人日间腔镜疝修补术中,采用喉罩全麻的方式可取得显著的麻醉效果,更好地维持围术期血流动力学稳定及降低术后相关并发症的发生率,特别是对日间手术患者更具优势,值得临床推广和使用。Objective To compare the anesthetic effect of general anesthesia with laryngeal mask and tracheal intubation respec?tively in adult daytime laparoscopic hernia repair.Methods A total of 60 patients admitted to the hospital from January,2017 to De?cember,2018 who underwent daytime laparoscopic hernia repair were randomly divided into the observation group(n=30)and the control group(n=30).The control group received tracheal intubation general anesthesia,and the observation group received laryngeal general anesthesia.The anesthesia effect of the two groups was observed.Results There was no significant changes in hemodynamics during the perioperative period in the observation group,while patients in the control group had higher systolic blood pressure,dia?stolic blood pressure and heart rate immediately after tracheal insertion(T2)and tracheal extubation(T3)than those in the observation group at the same time(P<0.05).Postoperative extubation time,anesthesia time and propofol dosage were shorter in the observation group than in the control group(P<0.05).The total incidence of complications in the observation group was 3.33%,which was signifi?cantly lower than the total incidence in the control group,which was 20.00%(P<0.05).Conclusion In adult daytime laparoscopic herniorrhaphy,laryngeal mask general anesthesia can achieve significant anesthetic effect,better maintain perioperative hemody?namic stability and reduce the incidence of postoperative complications,especially for daytime surgery patients with more advantages.So it is worthy of clinical promotion and use.
