后外侧入路与前外侧入路治疗肱骨中下段骨折临床对比研究

摘　　要：目的探讨在肱骨中下段骨折的治疗中,对比采用改良后外侧入路和前外侧入路方式治疗疾病的效果。方法选取惠州市中心人民医院2015年5月—2018年10月收治的60例肱骨中下段骨折患者为研究对象,随机分成实验组与对照组,每组30例。对照组行前外侧入路方式治疗疾病,而实验组则采取改良后外侧入路方式治疗,观察两组手术治疗效果及患者预后情况。结果手术情况上,在手术时间、术中出血量、术后下床时间及住院时间等指标上,实验组明显优于对照组,差异有统计学意义(P<0.05);随访6个月,在患者肘关节功能改善情况上,实验组功能优良率为96.67%,明显高于对照组80.00%的优良率,差异有统计学意义(P<0.05)。结论对于肱骨中下段骨折疾病,相比应用前外侧入路的手术方式,采取改良后外侧入路的手术方式治疗,具有手术时间短、术中出血量低、术后康复时间短及手术预后效果良好的特点,值得推广。Objective To investigate the efficacy of modified posterolateral approach and anterolateral approach in the treatment of middle and lower humerus fractures.Methods 60 patients with middle and lower humerus fractures admitted to the hospital from May,2015 to October,2018 were randomly divided into the experimental group and the control group.The control group were treated by anterolateral approach,while the experimental group were treated by modified anterolateral approach.The results of surgical treat?ment and prognosis of patients in the two groups were observed.Results In terms of operation,the experimental group was significant?ly less/shorter than the control group in terms of operation time,intraoperative blood loss,postoperative time out of bed and length of stay(P<0.05).In the 6-month follow-up,the excellent and good rate of elbow function in the experimental group was 96.67%,which was significantly higher than the 80.00%excellent and good rate in the control group(P<0.05).Conclusion Compared with the anterolateral approach,the modified anterolateral approach has the advantages of shorter operation time,lower intraoperative blood loss,shorter postoperative recovery time and better prognosis for humerus fracture.
