经玻璃体腔内注射雷珠单抗联合氩激光治疗糖尿病性黄斑水肿过程中的护理措施分析

摘　　要：目的研究经玻璃体腔内注射雷珠单抗联合氩激光治疗糖尿病性黄斑水肿(DME)过程中的护理措施效果。方法选取于2016年3月—2018年12月间在湛江中心人民医院接受经玻璃体腔内注射雷珠单抗联合氩激光治疗的DME患者92例作为研究对象。通过随机抽签法将其等分成研究组和对照组,对照组予以常规护理干预,研究组则予以综合护理干预。比较两组视力改善效果,干预前后组黄斑中心凹视网膜厚度(CRT)、最佳矫正视力(BCVA)、眼压水平变化情况,并发症发生情况,干预前后自我效能感水平变化。结果研究组视力改善总有效率高于对照组(P<0.05)。干预后研究组CRT、BCVA水平低于对照组(均P<0.05);而两组眼压水平对比差异不明显(P>0.05)。研究组与对照组眼内出血、高眼压、视网膜脱离、血管阻塞发生率对比均不明显(均P>0.05)。干预后研究组与对照组自我效能感评分高于对照组,且研究组高于对照组(均P<0.05)。结论经玻璃体腔内注射雷珠单抗联合氩激光治疗DME过程中的护理措施效果显著,有利于改善患者的视力,且不会增加并发症发生风险,改善患者的自我效能感水平,值得临床推广应用。Objective To study and analyze the effect of nursing measures in the treatment of diabetic macular edema(DME)by intravitreal injection of leizumab combined with argon laser.Methods 92 DME patients who received intravitreal leizumab injection combined with argon laser therapy in the hospital from March,2016 to December,2018 were selected as research objects.Through random lottery method,they were divided into study group and control group.The control group were given conventional nursing inter?vention,and study group were given comprehensive nursing intervention.The visual acuity improvement effects of the two groups were compared,including changes in retinal thickness(CRT),best corrected visual acuity(BCVA),intraocular pressure,complica?tions,and self-efficacy before and after intervention.Results The total effective rate of visual acuity improvement in the study group was higher than that in the control group(P<0.05).After intervention,CRT and BCVA levels in the study group were lower than those in the control group(all P<0.05).However,there was no significant difference in IOP level between the two groups(P>0.05).There was no significant difference in the incidence of intraocular hemorrhage,intraocular pressure,retinal detachment and vascular obstruction in the study group and the control group(all P>0.05).After intervention,self-efficacy scores of the study group were high?er than the control group,and the study group was higher than the control group(both P<0.05).Conclusion Intravitreal injection of leizumab combined with argon laser therapy for DME has a significant effect on nursing measures,which is conducive to improving the vision of patients,without increasing the risk of complications,and improving the level of patients’self-efficacy.It is worthy of clinical application.
