摘　　要：目的探讨老年患者同型半胱氨酸(Hcy)水平对胺碘酮转复心房颤动(房颤)成功率的影响。方法 70例持续性房颤的老年快速房颤患者,根据血清Hcy水平分为A组(37例, Hcy水平偏高)和B组(33例, Hcy水平正常)。两组均给予胺碘酮药物治疗,观察比较两组的复律效果及不良反应发生情况。结果治疗2周后, B组复律时间、QT间期、转复房颤成功率分别为(30.15±5.04)min、(0.54±0.06)s、78.8%,均优于A组的(47.53±6.20)min、(0.42±0.07)s、48.6%,差异均有统计学意义(P<0.05)。两组均无严重不良反应发生。结论老年房颤患者Hcy水平升高直接影响复律效果,控制Hcy水平是提高老年房颤患者胺碘酮转复房颤成功率的关键。Objective To discuss the effect of homocysteine(Hcy) level on the success rate of amiodarone in elderly patients with atrial fibrillation. Methods A total of 70 cases of elderly patients with persistent atrial fibrillation were divided by Hcy levels into group A(37 cases, high Hcy level) and group B(33 cases, normal Hcy level). Both groups was treated with amiodarone, and the cardioversion effect and occurrence of adverse reactions in the two groups was observed and compared. Results After 2 weeks of treatment, the cardioversion time, QT interval and success rate of cardioversion of atrial fibrillation were(30.15±5.04) min,(0.54±0.06) s and 78.8% in group B, which was better than(47.53±6.20) min,(0.42±0.07) s and 48.6% in group A, and the difference was statistically significant(P<0.05). No serious adverse reactions occurred in both groups. Conclusion The increase of Hcy level in elderly patients with atrial fibrillation directly affects the effect of cardioversion. Controlling Hcy level is the key to improve the success rate of cardioversion of atrial fibrillation in elderly patients with atrial fibrillation.
