多奈哌齐合并奥氮平治疗以精神行为症状首发的阿尔茨海默病患者的临床效果评价

摘　　要：目的探讨多奈哌齐合并奥氮平治疗以精神行为症状首发的阿尔茨海默病患者的临床效果。方法 60例以精神行为症状首发的阿尔茨海默病患者,随机分为多奈哌齐治疗组和多奈哌齐与奥氮平治疗组,每组30例。多奈哌齐治疗组患者采取多奈哌齐治疗,多奈哌齐与奥氮平治疗组患者采取多奈哌齐联合奥氮平治疗。比较两组患者的治疗效果,治疗前后社会功能、生理活力、情感职能评分及认知功能评分、神经精神问卷(NPI)评分。结果多奈哌齐与奥氮平治疗组患者总有效率为100.00%,高于多奈哌齐治疗组的70.00%,差异具有统计学意义(P<0.05)。治疗后,多奈哌齐与奥氮平治疗组患者社会功能、生理活力、情感职能评分均高于多奈哌齐治疗组,差异具有统计学意义(P<0.05)。治疗后,多奈哌齐与奥氮平治疗组患者认知功能评分(27.57±2.21)分高于多奈哌齐治疗组的(24.78±2.25)分, NPI评分(11.23±1.61)分低于多奈哌齐治疗组的(14.70±2.21)分,差异均有统计学意义(P<0.05)。结论多奈哌齐与奥氮平治疗以精神行为症状首发的阿尔茨海默病效果好,可更好地改善患者的精神状况、生活质量、认知功能和社会功能。Objective To discuss the clinical effect of donepezil combined with olanzapine in the treatment of patients with Alzheimer’s disease starting with mental behavioral symptoms. Methods A total of 60 cases of Alzheimer’s disease starting with mental behavioral symptoms were randomly divided into donepezil group and donepezil combined with olanzapine group, with 30 cases in each group. Donepezil group was treated by donepezil group, and donepezil combined with olanzapine group was treated by donepezil combined with olanzapine. Comparison was made on therapeutic effect, and scores of social function, physiological activity, emotional function, cognitive function and neuropsychological inventory(NPI) before and after treatment between the two groups. Results The total effective rate was 100.00% in donepezil combined with olanzapine group, which was higher than 70.00% in donepezil group, and the difference was statistically significant(P<0.05). After treatment, the cores of social function, physiological activity, emotional function in donepezil combined with olanzapine group was higher than those in donepezil group, and their difference was statistically significant(P<0.05). After treatment, the cognitive function score(27.57±2.21) points in donepezil combined with olanzapine group was higher than(24.78±2.25) points in donepezil group, and NPI score(11.23±1.61) points was lower than(14.70±2.21) points in donepezil group. Their difference was statistically significant(P<0.05). Conclusion Donepezil and olanzapine shows good effect in the treatment of Alzheimer’s disease starting with mental behavioral symptoms, and it can better improve the mental status, quality of life, cognitive function and social function.
