香砂六君丸对慢性肾衰竭脾肾气虚证疗效及胃肠激素影响

摘　　要：目的:观察香砂六君丸对慢性肾衰竭(CKD 3~4期)中医辨证属脾肾气虚证患者的临床疗效以及对胃肠肽激素的影响。方法:选取120例我院收治的确诊为慢性肾脏病3~4期、中医辨证属脾肾气虚证患者,所有患者均给予基础疾病的常规治疗,然后按照随机数字表法将患者分为两组,对照组给予口服莫沙必利,观察组给予香砂六君丸口服,疗程为4周,监测患者的中医证候积分、血肌酐(Scr)、尿素氮(BUN)、内生肌酐清除率(Ccr)、胃泌素(gastrin,GAS)、胃动素(motilin,MTL)及生长抑素(somatostatin,SS)变化。结果:治疗后,观察组各项证候积分显著低于治疗前(均P<0.05),且显著低于对照组治疗后(均P<0.05);血肌酐(Scr)、尿素氮(BUN)下降水平、内生肌酐清除率(Ccr)上升水平均显著优于本组治疗前及对照组治疗后(均P<0.05);胃泌素、胃动素、生长抑素水平显著低于本组治疗前及对照组治疗后(均P<0.05),而对照组仅胃动素显著低于治疗前(P<0.05)。结论:香砂六君丸能有效缓解慢性肾衰竭(CKD 3~4期)脾肾气虚证患者的症状,改善肾功能,降低胃泌素、胃动素、生长抑素等激素水平。Objective:To investigate the influnece of Xiangsha Liujun Pill to the clinical efficacy and gastrointestinal hormones in the treatment of chronic renal failure(CKD 3~4 stage)of qi deficiency of Spleen and Kidney.Methods:120 cases met the inclusion criteria were randomly divided into the control group and the observation group.The control group was treated with Mosapride Citrate;whereas the observation group was treated with Xiangsha Liujun Pill.The course of treatment was 4 weeks.TCM symptom scores,the changes of serum creatinine(Scr),urea nitrogen(BUN),endogenous creatinine clearance rate(Ccr),gastrin(GAS),motilin(MTL)and somatostatin(SS)were observed in the two groups.Results:After the treatment,the scores of TCM symptom were significantly lowered in the two groups(P<0.05);of which the observation group was lower than the control group(P<0.05).The decrease of Scr and BUN,and the increase of Ccr were more significant in the observation group than those in the control group(P<0.05).The decrease levels of GAS,MTL and SS were also significant in the observation group than those in the control group(P<0.05).Conclusion:Xiangsha Liujun Pill can effectively improve the symptoms and renal function,and reduce the serum levels of GAS,MTL and SS when treating chronic renal failure(CKD 3~4 stage)of Qi deficiency of Spleen and Kidney.
