血清Th1/Th2细胞因子与妊娠期糖尿病胰岛素抵抗的相关性

摘　　要：目的根据妊娠期糖尿病患者血清中6种Th1/Th2细胞因子的变化,探讨血清中Th1型、Th2型细胞因子与胰岛素抵抗的关联。方法将42例妊娠期糖尿病孕妇作为病例组,同期纳入37例健康孕妇作为健康对照组。通过流式细胞仪测定两组孕妇血清中肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、干扰素-γ(interferon-γ,IFN-γ)、白介素-2(interleukin-2,IL-2)、白介素-4(interleukin-4,IL-4)、白介素-6(interleukin-6,IL-6)、白介素-10(interleukin-10,IL-10)细胞因子含量。结果病例组血清中TNF-α高于健康对照组,IL-4低于健康对照组,差异具有统计学意义(P <0. 05)。同时两组间血清中IL-2/IL-4、TNF-α/IL-4、TNF-α/IL-10及IFN-γ/IL-4的比值,差异有统计学意义(P <0. 05)。结论妊娠期糖尿病患者血清中肿瘤坏死因子-α、IL-4含量发生异常,同时Th1/Th2细胞因子比值发生变化,Th1/Th2细胞因子参与了胰岛素抵抗。Objective To explore the correlation between Th1 and Th2 cytokines in serum and insulin resistance in patients with gestational diabetes. Methods Forty-two pregnant women with gestational diabetes were enrolled as case group,and 37 healthy pregnant women in the same period were chosen as healthy control group. The serum contents of tumor necrosis factor-alpha( TNF-α),interferongamma( IFN-γ),interleukin-2( IL-2),interleukin-2( IL-4),interleukin-6( IL-6),interleukin-10( IL-10) were determined by flow cytometry. Results The serum level of TNF-α was higher in case group than in healthy control group,while the IL-4 was lower( both P < 0. 05). There was no significant difference in levels of IFN-γ,IL-2,IL-6 and IL-10 in serum between two groups( P > 0. 05). At the same time,the ratios of IL-2/IL-4,TNF-α/IL-4,TNF-α/IL-10 and IFN-γ/IL-4 showed statistically different between two groups( P < 0. 05). Conclusion The levels of TNF-α and IL-4 are abnormal in serum of patients with gestational diabetes,and the ratio of Th1/Th2 cytokines has obvious changes. Th1/Th2 cytokines may be involved in insulin resistance of gestational diabetes.
