GDM产妇分娩的新生儿发生低血糖的产科因素及护理对策探究

摘　　要：目的:研究妊娠期糖尿病(Gestational Diabetes Mellitus,GDM)产妇分娩的新生儿发生低血糖的产科因素以及护理对策探究。方法:选取2017年11月-2018年11月我院接收的120例妊娠期糖尿病产妇,根据其新生儿是否发生低血糖(<2.2 mmol/L)将其进行分组,其中将发生低血糖的新生儿定义为研究组(41例),将未发生低血糖的新生儿定义为参照组(79例)。对其临床资料进行回顾性分析,观察妊娠期糖尿病产妇分娩的新生儿发生低血糖的产科因素,同时总结护理对策。结果:研究组与参照组产妇血糖控制效果、是否早产儿、是否低体温儿、是否低体重儿以及喂养合理性比较差异有统计学意义,P <0.05;研究组与参照组一般资料比较无显著差异,P> 0.05。经Logistic回归分析,产妇血糖控制效果差、早产儿、低体重儿、低体温儿以及喂养方式不合理为新生儿发生低血糖的产科危险因素(P <0.05)。结论:产妇血糖水平控制效果与新生儿是否早产儿、低体重儿以及低体温儿、喂养合理是妊娠期糖尿病产妇分娩新生儿低血糖发生的影响因素以及危险因素,制定并实施针对性护理干预措施,对降低妊娠期糖尿病产妇分娩新生儿低血糖发生风险的效果较为理想。Objective: To study the obstetric factors and nursing strategies for hypoglycemia in newborns from GDM puerpera. Methods: 120 pregnant women with gestational diabetes mellitus were divided into the two groups according to whether their newborns had hypoglycemia(<2.2 mmol/L). The newborns with hypoglycemia were defined as the study group(41 cases), and the newborns without hypoglycemia were defined as the reference group(79 cases). The clinical data were retrospectively analyzed to observe the obstetric factors of hypoglycemia in newborns from GDM puerpera, and to summarize nursing strategies. Results: Logistic regression analysis showed that maternal blood glucose, low birth weight infants, hypothermia infants, rationality of feeding methods and premature infants were obstetric risk factors for neonatal hypoglycemia(P<0.05). Conclusion: The control effect on maternal blood glucose and neonatal preterm infants, low birth weight infants, hypothermia infants and reasonable feeding are the influencing factors and risk factors for neonatal hypoglycemia in gestational diabetes parturients. The formulation and implementation of targeted nursing intervention measures can reduce the risk of neonatal hypoglycemia in gestational diabetes parturients.
