盐城市儿童疑似预防接种异常反应个体补充保险实施评价

摘　　要：目的评价盐城市儿童疑似预防接种异常反应(AEFI)个体补充保险的试点效果。方法在全市各县选取适龄儿童家长开展问卷调查,分析儿童AEFI个体补充保险的投保率、理赔、家长态度和满意度;选择所有接种单位人员开展问卷调查,分析其对AEFI补偿机制的态度和满意度。结果盐城市2016-2018年儿童AEFI个体补充保险的投保率为42.48%(861/2027);各年保险理赔金额(例数)分别为12.67万元(20例)、89.00万元(54例)、132.58万元(118例),理赔率分别为1.18%、7.59%、20.51%。儿童家长认为应降低保费(32.89%)和提高保额(31.81%)。97.17%(103/106)已理赔儿童家长对理赔满意。在496名接种单位人员中,61.29%认为财政基础补偿加商业补充保险为最佳补偿模式,93.36%认为个体补充保险的保费基本合理或合理。结论盐城市AEFI个体补充保险试点取得明显成效;应鼓励保险公司推出保费更低、保额更高的保险产品。Objective To evaluate a pilot project of individual supplementary insurance f or children with adverse events following immunization(AEFIs)in Yancheng city.Methods We selected parents of age-eligible children for a questionnaire-based survey of AEFI individual supplementary insurance to analyze insurance coverage,compensation,and parents’attitudes and satisfaction.We also conducted a survey among vaccination staff for an analysis of their attitudes toward and satisfaction with the AEFI compensation system.Results The coverage rate of AEFI individual supplementary insurance f or children was 42.48%(861/2027)in Yancheng during 2016-2018.The compensation amounts(numbers of cases)in the three years were 12.67 ten-thousand yuan(20 cases),89.00 ten-thousand yuan(54 cases),and 132.58 ten-thousand yuan(118 cases),with compensation rates of 1.18%,7.59%,and 20.51%,respectively.Parents suggested to reduce the purchase f ee(32.89%),and increase the compensation amounts(31.81%);97.17%(103/106)of parents who had received compensation were satisfied with the compensation.Of 496 vaccination staff,61.29%considered the f inancing-based compensation plus commercial supplementary insurance as the best model,and 93.36%considered that the purchase price for individual supplementary insurance was reasonable.Conclusions The pilot implementation of the AEFI individual supplementary insurance was remarkably effective in Yancheng.We should encourage the insurance companies to introduce lower-price and higher-compensation insurance products.
